We are  seeking an Advanced Clinical Practitioner to join our team at Skylark Rehabilitation Unit  including GP’s with special interest in  frailty and  Advanced Practitioner. 

The unit consists of 30 beds based at Skylark Rehab Unit in Yate, which provides an environment where individuals who are medically stable can receive rehabilitation assessment. The multi-disciplinary team works collaboratively to achieve safe, effective care for our patients whilst they aim to recover from hospital admissions and return home.

Main duties of the job

You will primarily work as part of our  medical team, responsible for the clinical care of our 30 patients.

Patients are admitted to Skylark Rehabilitation Unit for rehabilitation and recovery following an acute hospital admission. 

Throughout their admission, patients require medical intervention to:

  • Continue the management of their health needs and underlying conditions.
  • Optimise the patient medically to maximise their rehabilitation potential.
  • Utilise the multidisciplinary workforce to enhance the patients overall health. 
  • React and provide urgent medical intervention to manage new medical deterioration.

You will be responsible for the ongoing management of the patient’s current cause of admission  and other co-morbidities and supporting the wider multi-disciplinary workforce in delivering safe and effective care including rehabilitation of the patient to allow their recovery and return home.

  • Assessment and clerking of new patients admitted onto the unit.
  • Writing drug charts to allow safe administration of medications.
  • Noting and acting upon important and outstanding medical events from the patient’s admission into secondary care.
  • Working with collaboratively with colleagues at our acute partner hospitals  and  our sytsem partners.

Sirona care & health currently recruit overseas nursing and therapy staff through an NHS Trust Supplier and we adhere to the NHSE Code of Practice in relation to applicants from Red List countries. This post is not an Advanced Practice role that is mapped to the HEE Multi-professional Framework for Advancing Practice. These posts are currently under review within Sirona and are likely to undergo a name change in the near future. This is because these roles do not require candidates to work across all 4 pillars of Advanced Practice to MSc level and are therefore banded at AFC Band 7. They do, however require a skillset working at or towards MSc level in the Advanced Practice Clinical Pillar. This role will not be eligible for training funding for a full MSc in Advanced Practice however training may be supported on an individual basis depending on service and individual learning needs, following Sirona CPD funding application processes. Person specification

Skills, knowledge and experience

Essential criteria
  • Advanced skills and experience of managing service users with long term conditions and urgent care needs
  • Experience of using clinical examination skills
  • Development in professional and management procedures within own area and across several related disciplines relevant to service area.
Desirable criteria
  • Experience of working at Advanced Practitioner level within a health care setting

Qualifications

Essential criteria
  • Current registration with NMC or HCPC
  • Independent Prescribing Non-Medical Prescribing Qualification
  • Evidence of post-qualification/registration modules relevant to the core areas of Advanced Practice
  • Evidence of continuing professional development
Desirable criteria
  • Recognised post graduate management / leadership qualification, working towards one or willingness to undertake.
  • Masters level 7 Advanced Practice/completed Advanced Practitioner Pathway
