Job overview

We are looking to recruit an experienced and motivated senior substance misuse worker to enhance our clinical offer within our new co-occurring dual diagnosis service. Under the wider substance misuse team, this is a brand new opportunity to join a developing service as a harm reduction practitioner, specifically supporting individuals with co-occurring mental health and addiction needs.

Alongside your clinical team you will be expected to deliver a range of interventions which will include referral management, screening assessment, triage and evidence based interventions by providing one to one and group facilitated sessions whilst managing a mixed and challenging caseload. There will also be the expectation on the practitioner to perform some clinical tasks when required, though full training would be provided.

Main duties of the job

Duties will include performing robust assessment including risk, screening and interventions to offenders with co-occurring needs and ensure that all waiting times, assessments and interventions are delivered in a timely manner. Using specialist psychologically informed practice you will need to identify a range of direct and indirect interventions which will support service users to reduce the harmful impact of their drug and/or alcohol use, and support their recovery. Therefore experience of working with people with serious mental illness and multi-agency working will be essential as well as the ability to coproduce care plans and the willingness to think creatively and utilise a range of engagement skills to achieve positive outcomes.

This new role will focus on delivering patient centered care and support to a range of service users and you will ensure integrated and collaborative working such as community mental health teams and community drug teams to ensure appropriate sharing of information. There may also be occassions whereby you will need to provide comprehensive and timely reports and liaise with police, probation services, solicitors and court liaison teams as directed.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
