Main duties of the job

As a primary care HCSW you will support a clinician with a clinical caseload of men ensuring appropriate assessment and treatment interventions are provided and SMART goals are agreed.
You will:-
•    Contribute to and deliver interventions and/or treatments when there are complex issues and/or serious illness. You will discuss the treatment plan and the related needs with the service user and the wider team and will understand his / her own role in delivering interventions and / or treatments within the plan.
•    Identify any specific precautions or contra-indications to the proposed interventions /treatments and take the appropriate action.
•    Support and monitor patients throughout, promptly alerting the line manager when there are unexpected changes in individuals’ health and wellbeing or level of risk.
•    Provide information to the primary care team on how individuals’ needs are changing and feedback on the appropriateness of the individual’s treatment plan when there are issues.
•    Respond to, record and report any adverse event or incident relating to the intervention /treatment with an appropriate degree of urgency.
•    Support, undertake and record interventions / treatments correctly, and in line with legislation, policies and procedures and / or established protocols.
•    Under the supervision of a registered nurse, administer medication.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

The Job Requirements

  • You will have experience as a support worker with a real passion for developing your clinical and wider skills within primary care in an environment that is ever changing.
  • You’ll have a flexible approach working within a team that encourage new ideas and opinions on how to improve the service.
  • You will show initiative when it comes to handling a wide range of situations
  • You will hold an NVQ 3 in  a Health and Social Care related subject
