An exciting opportunity has arisen to join the Hospice at Home team (H@H). Working alongside the multidisciplinary team the H@H team provide high quality palliative and end of life care to patients wishing to remain in their own home.
We are inviting applications from candidates with a passion for palliative care and end of life care, looking for career development in this area. Candidates will need to work flexibly within the H@H service providing social, emotional, psychological and wider well-being care to our patients and their families.

- completion of application form relevant to post
- evidence of understanding of the role of an HCA in a Hospice at Home team
- evidence of team working
- evidence of working autonomously
- NVQ level 2/3 or equivalent
Desirable criteria
- Hospice at Home HCA experience
- communication skills training or evidence of courses/study
