Job overview

The Senior Practice Nurse will practice independently and autonomously delivering care and treatment to patients. The postholder will have gained a wide range of knowledge and clinical skills in a Primary care setting.

The role delivers a high standard of care on the treatment of minor illness and minor injury and the postholder has expertise in the management of long term conditions such as Diabetes, COPD, asthma, heart disease and support lead of female cancer screening services.

The Senior Practice Nurse will be experienced in the delivery of Public Health England’s health promotion programmes such as immunisations, women’s health, and lifestyle conditions such as smoking and obesity.

The postholder will have excellent organisational and time management skills to ensure clinics are managed efficiently and effectively, without compromising patient care or safety.

The Senior Practice Nurse will have line management and leadership responsibilities for the nursing team and lead on the practice based placements for students ensuring a safe and effective learning environment is maintained.

Main duties of the job

Clinical Skills:
1. Clinically assess, examine, diagnose, plan and evaluate treatment / interventions and care for patients presenting with an undifferentiated diagnosis
2. Non-medical prescriber (or working towards) who will prescribe and review medications within the practitioner’s competency and scope of practice for therapeutic effectiveness, appropriate to patients needs and in accordance with evidence-based practice. To ensure safe prescribing decisions at all times and only prescribe within scope of practice.
3. Monitor the delivery of care to ensure the individual needs of the patients are met in a timely manner.
4. Provide information to patients regarding their treatment options, assessments, investigations and follow up appointments, utilising effective verbal and non-verbal communication skills to promote understanding and avoiding confusion.
5. To communicate sensitively and articulately when explaining complex information or providing clinical advice when dealing with an emotional, highly charged situation such as a safeguarding concern, bereavement, or the diagnosis of a terminal illness.

Detailed job description and main responsibilities

6. Promote the development, implementation and review of standards, policies, procedures, and clinical guidelines.
7. To ensure a high standard of documentation at all times, recording information accurately and in a timely way in patients records.
8. Act as a role model promoting and demonstrating the standards of care expected and challenging poor practice.
9. Effectively motivate staff in the provision of care and future service development.
10. Promote evidence based clinical practice and be responsible for continuous improvement.
11. Ensure clinical governance encompasses quality improvement that focuses on the fundamentals of care.
12. Have current clinical experience of working in an acute medical environment and be competent in the associated skills required.
Management and Leadership:
13. Encourage and support staff within their working practice, maintaining confidentiality.
14. Demonstrate effective communication with all staff ensuring dissemination and exchange of information.
15. Listen and respond appropriately to patient feedback and manage complaints effectively in accordance with Spectrum’s policy.
16. Attend meetings and act as a representative of the department, Head of Healthcare, business unit and Spectrum when required to do so.

17. Work collaboratively with the MDT to develop and maintain high standards of patient care.
18. Act as an expert resource for the care of patients with medical conditions.
19. Work within the NMC code of conduct.

20. Collaborate with colleagues to create a supportive, dynamic, smooth flowing working environment.
21. Act as a visionary to inspire and motivate staff.
22. Develop effective links with universities and training facilities as practice facilitator
23. Ensure effective oversight is maintained to support clinical risk management for clients and staff
Service Development:
24. Work with all members of the team to evaluate patients experience of the service provision, make recommendations and implement actions to improve services.
25. To been influential in recording QoF data which underpins financial reward but also ensures the provision of safe effective care to the patients.
26. Liaise with wider clinical team to identify areas of change in practice and ensure these are achieved.
27. Demonstrate effective leadership by supporting and motivating staff within a changing environment.
28. Assist in the development of protocols and competencies to reflect service development.
29. Actively participate in the delivery of nurse led clinical interventions.
30. Contribute to the on-going development of the nursing team and the service
31. Demonstrate a clear, up to date knowledge and understanding of National, Spectrum and Prison issues and priorities, in particular the need to support proactive patient flow.

Person specification

Education

Essential criteria
  • Registered Nurse/1st Degree
  • NMC registered.
  • Post registration Level 3 or 4 qualification in history taking and physical examination skills
  • Non-Medical Prescriber or prepared to work towards
  • Evidence of relevant up to date CPD
  • Diploma in a Long-Term Condition management eg. asthma / diabetes
  • Recent or current experience in Primary Care
  • Experience of developing and delivering training
Desirable criteria
  • Teaching and Assessing qualification
  • Minor illness and minor injury accredited course Level 3 or 4
  • Accredited Family Planning/ sexual Health qualification.
