Job overview

To support the Medication Safety Officer (MSO) in investigating medicines incidents, reviewing investigations, disseminating alerts, preparing reports, and sharing lessons learnt.
To support all aspects of the medicine’s safety agenda across Bradford District Care NHS Foundation Trust.
To ensure consistent review and updating of medication errors.
To act as an advisory on medicines safety.
To support the MSO in disseminating lessons learnt from incident investigations, to identify medicines safety issues from national guidance and embed into clinical practice.
To assist in preparation for the Medicines Management and Safety Group.
To analyse data, providing interpretation and reports as appropriate to a wide variety of staff groups.
To provide training on medicines safety to a wide variety of staff groups

Main duties of the job

Key Responsibilities:
• To liaise with services to ensure medication errors are reported and investigated appropriately.
• To disseminate medication alerts and coordinate response.
• To train staff in incident reporting and investigation.
• To ensure that medicines management practices within MDT and services comply with legal, national, and local guidelines.
• To ensure that drug therapy is evidence-based, safe, cost-effective, and appropriate to the individual patient.
• To support the Trust with the implementation of the patient safety incident response framework for responding to patient safety incidents for the purpose of learning and improving patient safety.

Qualifications

Essential criteria
  • Holds professional clinical registration as Nurse / Pharmacist / Pharmacy Technician
  • Leadership, management, and quality improvement experience and/ or qualification.
Desirable criteria
  • Post graduate clinical qualification

Training

  • Evidence of regular Continuing Professional Development.
  • Awareness of performance measures (specifically with reference to Clinical Governance and Healthcare Standards)

  • Supervision of staff
  • Delivery of training
  • Experience of multidisciplinary working
  • Experience in educating patients and carers
  • Medicines management
  • Relevant professional, managerial and leadership experience of pharmacy technicians and support staff.
  • Excellent time management skills required to manage own time most effectively by planning and prioritising workload appropriately.
  • Use judgement to respond to service/ staffing changes to maintain services and ensure a safe working environment within the team, also regarding when to escalate any concerns.

Knowledge

Essential criteria
  • Have a knowledge of clinical governance, clinical audit, and risk management.
  • Have up to date knowledge of mental health and community services.
  • Have knowledge of primary and secondary care services.
  • Understanding of health services and medicines/ pharmacy related issues in primary and secondary care.
  • To have knowledge/ experience of quality improvement and service development
Desirable criteria
  • Understanding of local and national priorities through thorough knowledge of the NHS, government and local structure, policy, process and guidelines relevant to medicines management, pharmacy and mental health.
