Job overview

We have an exciting opportunity for a dynamic and experienced Health and Fitness Assistant to join our Physical Health and Wellbeing Team. 

The role will join an established and highly experienced team to support people under the care of the Trust to improve their health. The Trust is committed to delivering the Bradford District and Craven action plan to reduce premature mortality in people with severe mental illness (SMI) and this role is an essential part of this plan. 

We aim to improve health by supporting people to increase their physical activity by proving person centered care. 

We improve the health of those at risk by identifying them earlier, improve the health of those diagnosed by empowering them to take an active role in managing their health by supporting the social, physical and emotional needs of patients.

Main duties of the job

The post holder will work under the clinical supervision from the Health Improvement Lead and Allied Health Professionals (AHPs) to implement physical activity intervention plans and other activities which promote service user health and wellbeing both within inpatient settings and in community. 
The post holder will be responsible for devising safe exercise/ fitness sessions with support from the Health Improvement Lead. 
The post holder will be responsible for providing timely and accurate written and verbal feedback for all interventions as part of an overall physical health plan. 
The post-holder is responsible for working collaboratively with other Trust services to provide seamless mental health care for service users and seeking supervision to ensure understanding of occupational performance, communication, and impact of the mental illness upon the service user. 
To provide practical support, spend therapeutic time, including, using prescribed exercises to maintain / improve the physical wellbeing of an allocated group of service users with mental health needs within a secure setting. 
To promote service user’s recovery through the delivery of high quality, outcome measured care and evidence-based interventions. 
To assist AHPs and Health Improvement Lead in the implementation and evaluation of individualized care plans.

Essential criteria
  • Sports Instructor/ Fitness qualification at Level 2 or willingness to complete training.

Experience

Essential criteria
  • Relevant experience working within health, social care settings which provides transferable knowledge and skills
Desirable criteria
  • Experience of working with people with Mental Health needs and managing their physical health/ fitness as a member of a multidisciplinary team

Knowledge and Skills

Essential criteria
  • Knowledge and skills around physical health promotion/ ability to offer lifestyle advice based on UK guidelines.
  • Ability to deliver a suitable exercise/ fitness programme as part of a physical health care plan.
