Job overview
We have a great opportunity for a Band 6 Nurse as a Triage practitioner for our Specialist Older Adults Service within High Weald Lewes and Havens Assessment and Treatment Service.
If successful, you would join a dedicated and dynamic team, and would be fully supported and empowered to make a difference to creatively develop many new ways of working towards building safe and responsive quality mental health services fit for the future.
The job role entails ensuring referrals into our Older Adults service are triaged and managed within the agreed time scales and appropriate to clinical need. You will need to possess excellent assessment skills, as well as having working knowledge of mental health services.
As this post is at the front end of our service delivery, you will need to have excellent communication and interpersonal skills to enable you to build trusting relationships with our partners.
This post offers the Trust's £2,000 Golden Hello or Relocation Incentive for Band 5-6 Nurses & Practitioners, (pro rata and subject to terms and conditions; please see attached Protocol Document for full details).
The post is 22.5 hours per week and is based at Hill Rise, Newhaven, East Sussex.
Main duties of the job
Main Duties of the Role:
- Review referrals from other professionals for level of appropriateness, urgency and risk; and re-direct and sign-post to other services if necessary.
- Gather information relevant to referrals from appropriate sources such as GPs and adult social care.
- Provide a point of contact for patients initially referred into the service.
- Make initial contact with patients referred into the service to discuss their current presentation, risk and care pathway.
- Support carers and families, and engage with carers to provide information relevant to the referral as part of the team's approach to triangle of care.
- Attend team meetings weekly to discuss complex and risky referrals within an MDT setting.
- Be involved in raising safe guardings when there are concerns about vulnerable adults or children at risk.
