Community Mental Health Practitioner - Band 6, Full time, permanent position 

Hill Rise Assessment and Treatment Service in Newhaven is part of a lively multi-disciplinary service for adults with severe mental illness. Newhaven is a harbour town sitting within the beautiful landscape of the South Downs National Park and only a few miles from the cosmopolitan city of Brighton. Over the next five years our community mental health services are undergoing a huge transformation as we move to a more integrated network of services, with our mental health teams being central in driving the changes.

We are looking for a qualified Social Worker, Occupational Therapist or Nurse for this post. We can offer excellent B5-6 development opportunities for newly qualified professionals in either of these disciplines.

The role of a Community Mental Health Practitioner is to work in partnership with service users who may have severe and complex mental health problems to , develop care plans to enable them to lead fulfilling lives. They will demonstrate an understanding of clinical interventions relevant to this care group and be able to work closely with carers, families and partner agencies to facilitate this. The post holder will work at all times to promote the safety and the well being of service users and their families/carers. The post holder will assess, plan and implement care, provide specialist advice and carry out specialist interventions relevant to their discipline. They will provide clinical supervision to staff and students as appropriate and ideally be able to act as a mentor or practice educator or students.

  • Registered Practitioner with current registration (mental health/ Adult/ Social Work/ OT) appropriate to the role
  • Evidence of continuous professional development

Knowledge & Experience

  • Significant post qualifying experience (minimum two years) in relevant care group
  • Experience of working alongside people with psychosis and complex health and social care problems in the designated care group
  • Experience of providing a range of clinical interventions to people in the designated care group with a variety of health problems
  • Experience of working consultatively with professionals
  • Experience of conducting clinical assessments including risk assessments
  • Experience of partnership working with in CPA process
  • Understanding of clinical governance
