A fantastic and exciting opportunity has arisen for a motivated, experienced and enthusiastic individual to work within the Clinical Governance team as a Patient Safety Incident Lead.

Would you like to be part of a dynamic and highly motivated team to improve patient care, whilst ensuring effective working relationships across the trust. The ideal candidate must have a flexible proactive attitude as they will work alongside clinical teams across organisational and professional boundaries to support the delivery of high-quality patient centred care.

Your primary responsibility will be to undertake Patient Safety Incident Investigations (PSIIs) underpinned by PSIRF methodology. You will be supporting other PSII investigators and will support Divisions throughout the Organisation. You should have experience and understanding in all aspects of patient safety, human factors and incident investigation or review, with demonstrable organisational skills and be able to work autonomously and as part of a team. Previous experience of working within a health care setting would be essential.

In order to be a successful Serious Incident Lead in our team you will:

  • Hold a clinical qualification
  • Be responsible for investigating PSIIs and with the Division oversee the reporting process.
  • Provide recommendations and advice for the future safety, treatment, care and well-being, of people who use our services, their families, carers and staff
  • Maintain records and data undertaking analysis and provide reports
  • Manage sensitive and contentious investigations dealing with highly emotional situations with Service Users, carers and families, staff and partner agencies
  • Be required to give evidence at inquests and other formal investigations
  • Work with Practitioners and Teams to test policy, practice and procedure in order to reduce the risk of harm and repeated incidents.
  • Reduce delays in the PSII investigation process and ensure that the process is concluded to a high standard within the agreed time frame.
  • This post requires intense intellectual concentration and interpersonal effort as the post holder will be dealing with a large number of people working in different roles, and will be leading on complex and sensitive information. They will also be required to undertake a significant degree of travelling throughout the Trust’s area. These will be regular demands
  • Have the ability to analyse and interpret complex clinical situations and arrive at well thought out risk assessed decisions in short period of time

  • Extensive experience of working at a senior manager level at band 7a or above.
  • Experience of investigating SUI’s, disciplinary issues and complex complaints.
  • An awareness of national health policy and mental health policy, in relation to clinical governance as it impacts on the management of incidents.
  • Awareness of PSIRF and root cause analysis.
  • Investigation ‘Root Cause and Analysis’.
  • Experience of working in multi-disciplinary teams.
