Salary: £35,392-£42,618 per annum
Please note if you commence in post prior to October 2024, you will be paid in line with 2023/2024 agenda for change pay scales.

Main duties of the job
Working as part of a multi-professional team to deliver enhanced clinical practice to a specific client group and participate in education, audit and application of evidence-based practice within the specialty.
Specialist Practice
- Manage daily anticoagulation nurse led clinics
- Receive direct referrals from General Practitioners and Consultants
- Manage and co-ordinate all investigations and treatment for DVT patients from referral to diagnosis according to the DVT protocol
- Participate in approved Patient Group directions by the Trust appropriate to our sphere of practice, to maintain safe anticoagulation for patients.
- Co-ordinate patient discharge liaising with all members of the multidisciplinary team to ensure that safe delivery of anticoagulation continues as an outpatient

Detailed job description and main responsibilities
Carry out programmes of care and treatment determined by others.
Manages clinical caseload on a day-to-day basis including making minor modifications within prescribed parameters reporting back to the Clinical Nurse Specialist.
Provides advice in relation to the care of individual or groups of patients.
Deliver education programmes to patients, carers and other nursing staff as agreed by the Clinical Nurse Specialist.
Act as a role model exploiting at every opportunity to enhance the knowledge of others in issues relating to the specialism.
Participates in research and audit as part of the multi-professional team within the specialism.
Communicates effectively with colleagues, patients and other users of the Trust.
Produces written communications which are clear, fluent, concise and easily understood.
Manage daily anticoagulation nurse led clinics.

Experience and Knowledge
Essential criteria
- Two years at Band 5 or equivalent
- Evidence of continuing professional development.
- Demonstrates the use of evidence- based practice in the implementation of change.
Desirable criteria
- Experience within the speciality

- RGN / RSCN / RM
- Willing to work towards a post grad qualification in speciality.
