Job overview

An exciting opportunity to join One Coventry Integrated Team (OCIT) as a Clinical Practitioner has arisen. Clinical Practitioners support all Urgent Emergency Care (UEC) pathways within OCIT including crisis response as part of the UEC offer. The Clinical Practitioner  will work autonomously to deliver a high standard of patient care, utilising advanced clinical skills and in-depth theoretical knowledge. 

Main duties of the job

1.    To work as an autonomous practitioner to deliver a high standard of patient care as part of the One Coventry Integrated Team; using advanced clinical skills and in-depth theoretical knowledge base.

2.    To effectively manage a complex caseload of patients as part of a multidisciplinary team (MDT), providing specialist and evidence-based assessment and interventions to patients in both a stable and acute phase of their condition, ensuring the physical, psychosocial, behavioural, perceptual, cognitive, sensory and environmental needs of patients, are addressed.

3.    Utilising highly specialist knowledge and clinical reasoning, to plan and provide clinical intervention in consultation with other colleagues in the multidisciplinary team and to work in a range of settings as appropriate (e.g. patients home, work environment or other community setting).

4.    To signpost, refer and support access to health, social and third sector services to reduce inappropriate attendances and admissions at A&E and improve patient quality of life outcomes.

5.    To work with and through others to support staff in the process of change by using influencing skills in order to effect change. Contribute to plans that reflect changes in practice and service delivery.

6.    To monitor and evaluate practice through clinical audit and formal evaluation processes.

Detailed job description and main responsibilities

  • To work collaboratively within the multi professional team to plan and implement treatment for acute illness or complex chronic disease management as required.
  • To work collaboratively with other key partnership organisations to ensure effective, safe delivery and transfer of care.
  • To participate in 7 day working rotas to ensure delivery of key services.
  • Support the review and development of competencies as required supporting clinical practice.
  • Ensure all clinical records are kept in line with trust policies, procedures and guidelines.
  • Effectively manage short-term caseloads identifying when patients are ready for either discharge or care transfer to other service providers internal or external.
  • Provide senior clinical supervision, education support and leadership to the One Coventry Integrated Team and wider UHCW services.
  • Provide operational support for the management team within Integrated Community Services.
  • Develop and formulate a comprehensive plan of care ensuring the individual’s perspective and opinion are explored at all levels and their wishes clearly identified.
  • Monitor indicators of chronic disease, anticipating possible decline and proactively managing this to enhance well-being and maintain independence.
  • Plan interventions in light of patients expresses wishes and goals of care, facilitating discussion with the family and significant others concerning end of life care plans
  • Ensure a robust process is in place to review care treatment plans evaluate and review plans of care.
  • To triage referrals into the service and ensure care interventions are implemented from a variety of sources to meet the patient’s needs.
  • Assess risk of providing care within the community and implement the relevant mitigations to reduce/remove any identified risk
  • Accountable for own actions in accordance with the Code of Professional Conduct. Contribute to corporate objectives, acting within local, Trust and statutory guidelines and policies at all times.
