Band 6 – Community Practitioner – Forensic Community Service
Some exciting opportunities have arisen to be part of the Forensic Community Service.
We are actively seeking to recruit an experienced registered Mental Health or Learning Disability Nurse to join our diverse team. You will be supporting the service as one of our Community Practitioners
The Forensic Community Service is a specialist community team that has a fully embedded MDT – so the successful candidate will join a multi-disciplinary team that includes Medical, Occupational Therapy, Nursing, Social Work, Speech and Language Therapy and Psychology colleagues.
We are a regional service which provides care and treatment for adults (both male and female) who have a history of offending and/or pose a risk to others.
Our service users have come into contact with the criminal justice system and have been within the secure in-patient pathway. We provide Care Co-ordination for patients across the region, along with providing in-reach to patients within secure in-patient services.
Main duties of the job
The post holder will manage their own caseload contributing to the assessment process and carrying out interventions. They will work closely with the other members of the team, supporting patients through their recovery. We hope to recruit new staff who are dynamic in their approach and wish to be part of an exciting service. Ongoing training around specialist risk assessment tools will be offered to the successful candidates.
Desirable attributes for the successful candidate include previous knowledge or working with offenders and patients with complex needs.

The service currently operates during working hours of Monday to Friday (0900-1700). There is an option to condense your hours across four weekdays.
The team covers the whole of the Cumbria Northumberland Tyne & Wear NHS Foundation Trust footprint. There is also some working into Tees, Esk & Wear Valley NHS Foundation Trust as part of the Provider Collaborative.
Detailed job description and main responsibilities
The aim of our community services is to reduce length of stay in secure beds, reduce re-admission and provide an ongoing service in the community using interventions based on NHS England's '12 components of Care', a biopsychosocial model.
The team also provide assessment and expert advice to other Mental Health services regarding risk management which will enable patients to remain in the least restrictive setting and reduce the risk of offending/re-offending.

Certificates & Qualifications
Essential criteria
- Professional Qualification and current Professional Registration as, a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker or Clinical Psychologist
- Mentor Training.
- Evidence of post-registration training in clinical/ psychological interventions / skills relevant to the post.
- Proven track record of post registration clinical experience relevant to the post.
- Training in CPA (Care Programme Approach), Safeguarding, MAPPA (Multi Agency Panel for the Protection of Vulnerable Adults) and Child Protection.
- Up-to-date knowledge/experience of relevant legislation.
Desirable criteria
- Experience of working within a community team
Knowledge & Experience
Essential criteria
- 12 to 24 month band 5 qualified nurse experience dependent upon service need.
- Awareness and understanding of professional accountability, NMC standards and other local and national initiatives.

- Demonstrate effective written and verbal communication skills.
- Evidence of a high level of professional, organisational and leadership skills (self & others).
- Ability to adapt within a changing environment.
