Are you an established Clinical Nurse Specialist in Diabetes / Diabetes Specialist Nurse considering a new leadership role?

We are looking for an exceptional nurse leader who is an expert in Diabetes care and has experience working as a Diabetes Nurse Specialist.  This is a Health Board wide role, and is responsible for the planning and delivery of diabetes  diabetes educational programmes.  This will include the
ThinkGlucose educational programme across the Health Board.

The post holder will have a proven track record as a Band 7 CNS in Diabetes within the community and secondary care.

The post holder will be expected to exercise a high degree of personal professional autonomy and make critical judgments to satisfy the expectations and demands of the role.

Main duties of the job

At this level the post holder is expected to work as a highly skilled specialist nurse and to have  expert knowledge in order to lead on the implementation and evaluation of specialist diabetes focused education and training.

The post holder will be responsible for leading and coordinating audits and evaluations into the delivery and effectiveness of the training program, write and contribute to reports on effectiveness of the programme to Executive Board members when required

They will provide highly specialised professional advice, consultancy, clinical expertise and leadership to patients, carers and Health Board staff.

They will demonstrate advanced skills and competences to directly influence the delivery of programmes of care across the full range of care providers across the organisation

Detailed job description and main responsibilities

The post holder will be required to work autonomously, leading and interpreting the diabetes project ‘ThinkGlucose programme’ for sustainable application locally
They will coordinate and/or develop and provide standardised Health Board teaching material to provide training and education to all staff groups across acute, community and primary care settings of the Health Board
This will reflect priorities identified in the findings of National Diabetes Audits, Health Board assessments of compliance with National Service Framework
The post holder will require highly developed clinical skills in order to deliver training to all levels of employees and independent contractors across the Health Board
The post holder will work closely with front line diabetes teams, communications team to develop a communication and promotion programme to ensure the raised awareness of diabetes care, using the agreed framework. This communication programme will be effective for all staff and indeed all patients and be adapted for those patients with 
specific communication needs National links and networking will ensure that the work undertaken in the Health Board

Qualifications

Essential criteria
  • Registered Nurse Level 1 (Active NMC Registration)
  • Degree
  • Masters in relevant health related subject (Diabetes)
  • Highly specialist knowledge of Diabetes in clinical practice/post graduate qualification in Diabetes Care
  • n depth knowldege of National and local evidence based diabetes care
  • Recognised management qualification or equivalent demonstrable experience
  • Knowledge of quality and service improvement methodologies
  • postgraduate teaching and assessing qualification or equivalent
  • Knowledge and understanding of current policy and issues in professional pratice, compliance, standards and the wider NHS
  • Knowledge of Safeguarding Adults/Children including Mental Health Act and Mental Capacity Act and Deprivation of Liberty Safeguards

Experience

Essential criteria
  • Extensive previous experience in the management of adults with diabetes and fluctuating and complex diabetes conditions
  • Evidence of improving diabetes care
  • Proven track record of leading and achieving improvement in the management of people with diabetes
