Continuing Healthcare Clinical Lead

NHS Frimley Integrated Care Board was formed on 1 July 2022 under the Health and Care Act 2022, replacing NHS Frimley Clinical Commissioning Group.

The ICB is the new statutory NHS organisation responsible for planning and delivering health and care services. It will work collaboratively with partner organisations including the voluntary, community and social enterprise sector, people and communities across the Frimley Health and Care Integrated Care System (ICS).

NHS Continuing Healthcare (CHC) is a package of continuing care that has been arranged and funded solely by the NHS to meet a person’s physical and mental health needs arising from disability, accident or illness.  It is for vulnerable people with complex, intense and unpredictable needs or people who are rapidly deteriorating and possibly entering a terminal phase.

NHS Frimley ICB’s East Berkshire Continuing Healthcare Service is seeking to recruit a highly motivated, very well organised and experienced Registered Nurse with qualifications in LD, MH or General Nursing or an Allied professional i.e. Physiotherapist, Occupational Therapist, Social Worker to join the team of CHC assessors to support in the processing of Continuing Healthcare and Funded Nursing Care assessments, reviews and case management.  You will have knowledge of the National Framework for Continuing Healthcare (July 2022) and be able to:

Supporting and Management of Staff.

Day to day overseeing of the operations of business as usual within CHC Team.

Participating in an Independent Review Panels at NHSE.

Ratification of CHC assessments.

Liaising and participating in Non agreed Panels with Local Authority Partners.

Verifying Decision Support Tools completed by Nurse Assessors.

Case Management and reviews.

Fast Track clinical administration.

Participating in Local Resolution Meetings.

Support and care planning.

You will be a key member of the team and will need to demonstrate skills in IT, report writing, managing complex patients. You will also have excellent verbal and written communication and have the ability to work in stressful situations and be able to prioritise workloads to meet deadlines.  You will be working in partnership with our local authorities and experience of multi-disciplinary working is essential. 

A current driving licence and vehicle used for business use will be required.

Frimley ICB covers around 800,000 people across the following five areas:

The post holder will support the Continuing Healthcare Clinical Manager and the Continuing Healthcare Service as a whole team to deliver an effective and successful Continuing Healthcare Service.

The post holder will lead either the referral or case management team and will line manage a team of Continuing Healthcare Nurses.

The referral management team leader will be responsible for all referrals facilitating multi-disciplinary assessments in line with operating policy and procedures so that assessments and decisions are conducted in a professional and timely manner.

The case management team leader will be responsible for the care packages, and reviews of patients assessed as being eligible for CHC funding and funded nursing Care, in line with operating policy and procedures so that patients receive high quality safe care.

The post holder will ensure that they and their team adhere at all times to the relevant National Framework and guidance, NMC Code of Professional Conduct and work within established policies, procedures, protocols and systems ensuring quality standards and performance as directed by the Clinical Nurse Specialist and Operational Manager.

  • Educated to degree level in relevant subject or equivalent level qualification or significant experience of working at a similar level in specialist area.
  • Registered with the NMC/ HCPC
  • Post-graduate degree in Management Studies or equivalent
  • Significant experience of successfully operating in a politically sensitive environment
  • Evidence of continued professional development
  • Demonstrated experience of co-ordinating projects in complex and challenging environments
  • Experience of managing risks and reporting
  • Experience of drafting briefing papers and correspondence at SMT level

  • Demonstrated capability to act upon incomplete information, using experience to make inferences and decision making

  • Knowledge of Financial Systems e.g. monitoring budget management, processing invoices and procurement
