Job overview
To deliver nursing care in accordance with national and local policy and guidance and in accordance with the Nursing and Midwifery Council.
The post holder will be a Registered Nurse in the community ‘community nurse’ who works as part of a team to deliver nursing care and treatment autonomously as delegated by and under the direction of the District Nurse.
Main duties of the job
• The post holder will visit adult patients in their own home who have an identified nursing need including those with a long-term condition, complex care needs and needing care at home and the end of life.
• The post holder will use an electronic patient held record system, EMIS as a record of care and all our nurses are provided with a work Iphone and Ipad for mobile working.
• The community nurse will use a range of nursing skills including venepuncture, wound care, catheterisation, syringe drivers, end of life care and symptom control and on successfully attaining all the competencies required will be reviewed for a Band 6 Senior Community Nurse post.

Detailed job description and main responsibilities
We recognise that a highly skilled community nursing workforce is the cornerstone to safe patient care and avoiding unnecessary admission to hospital and have lots training and development opportunities available, alongside a career development pathway with opportunities for development at every level through to band 3 to band 8.
All community nurses work as part of a local team, closely with the local acute hospital, social care, mental health teams, GPs, and Practice Nurses and use the electronic clinical record, EMIS with a work Ipad and Iphone provided.

We are seeking a highly motivated Registered Nurse to work within a wider team of nurses and health care assistants. You will work under the direction of a District Nurse, to assess, implement and evaluate person centred care for a caseload of patients, undertaking venepuncture, wound care, catheterisation, syringe drivers, end of life care and symptom control. On achieving all relevant competencies, the post holder will be reviewed for a Band 6 Senior Community Nurse post.
Post holders will be available to work Monday to Sunday between 8 am and 8 pm and be able to travel to patients’ homes frequently throughout the day across the locality and across Guildford and Waverley.

Qualifications
Essential criteria
- Registered Level 1 Nurse with current Nursing and Midwifery Council registration
- Educated to diploma or level 6 / degree level in relevant area
- Supervisor / Mentorship qualification (within the first 2 years of post)
- Evidence of continuing professional development
- Area of special interest in the management and treatment of long-term conditions and willing to undertake additional education in this area
Knowledge
Essential criteria
- Knowledge of recent developments within health and social care
- keeps up to date with evidence-based practice and research related to nursing and health care in the community
- Ability to safely and effectively operate community equipment
- Ability to assess nursing needs and implement nursing care and treatment
- Ability to identify and pass on urgent client information to the District Nurse (or deputy senior community nurse)
- Ability to work across a seven-day week between 8 am and 8 pm as per service requirements
- Ability to prioritise visits in accordance with patient clinical need
- Ability to organise own workload without direct supervision
- Ability to teach and supervise students and team colleagues
- Able to teach patients and ensure they are supported with health promotion and self care
- Dexterity to enable clinical skills to be performed i.e. removal of sutures venepuncture IV Therapy
- Able to use databases and other IT programmes, including MicroSoft Word, Excel, Powerpoint, Emails, Virtual Meeting platforms
- Ability to input and navigate around the community electronic patient record system. EMIS (training will be provided)
