Diabetes Specialist Nurse

Job overview
The role is to provide specialist nursing for the care of the patient with diabetes, their families and other health care professionals.
This post will enable the applicant to develop their knowledge of diabetes and its management. The applicant will be supported to provide evidenced based care to ensure clinical outcomes that meet the challenging targets for the service.
Main duties of the job
· In conjunction with other members of the Diabetes Team, to provide specialist diabetes care to adults and young people who attend the Diabetes Clinics.
· To assess, plan, implement and evaluate holistic specialist nursing care in both the hospital and outpatient setting and maintain appropriate records for an individual caseload.
· Act as a source of specialist advice on management of diabetes, including the provision of basic dietary advice and the management of acute complications - either face to face or by telephone on a day to day basis.
· To provide individual specialist teaching to patients and carers to facilitate the development of self-care skills and knowledge, at the time of diagnosis.
· To facilitate discharge from hospital for newly diagnosed or pre-existing
Diabetes patients, arranging follow-up in clinic or by telephone as need dictates.
· To establish and maintain effective communication with patients with diabetes in order to facilitate appropriate responses / adaptation to life with a long term condition.
· Adjust insulin doses in liaison with the medical team and the senior clinical nurse specialist.
· To minimise hospital admission by providing advice/information and psychological support by Telephones, particularly during periods of inter current illness.

Detailed job description and main responsibilities
Clinical Management
· To use clinical judgement, in liaison with the diabetes medical team and Lead nurse, to make decisions about the care and management of patients on the clinical caseload.
· To contribute towards the development and expansion of the adult Diabetes Service to improve care for patients with diabetes to ensure local and national standards are met.
· To participate in clinical audit and research to identify areas of clinical practice requiring development.
· To participate in the formulation of policy, standards, protocols and guidelines and specialist
diabetes care delivery strategies and implement and evaluate such developments across the Trust.
· To participate in Network initiatives.
Teaching and Education
· To act as a resource and educator in diabetes to ward nursing and medical colleagues
· To contribute to the teaching of student nurses and other healthcare professional regarding the
care and management of diabetes - including participation in study days
· Demonstrate reflective practice skills and participate in clinical supervision.
Professional
· Act in accordance with NMC Code of Professional Conduct (2015).
· Maintain own professional development, including Trust Mandatory Training and Child
Protection training
· Regularly review and update own professional education and practice by undertaking specialist
training, attending meetings / conferences, networking with others working in the field of
diabetes and reading relevant journals.
· To participate in the staff appraisal scheme as an appraise and be responsible for complying
with agreed personal development plans to meet required level of knowledge and
competencies.
· To attend Diabetes Team Meetings and multi-professional meetings, and in the absence of
Lead Diabetes Specialist Nurses to represent the interests of the adult Diabetes Team.

Essential criteria
- Current NMC Registration
- Educated to degree level or equivalent
- ENB 998 or equivalent teaching/mentorship course
Desirable criteria
- Diabetes qualification
Experience

- Significant post registration experience
Desirable criteria
- Working with adults with diabetes in primary or secondary care

You must have appropriate UK professional registration.
