Job overview
Surrey and Borders Partnership are seeking to recruit a qualified OT or Nurse to work as part of our Integrated Care Team within primary care, supporting people 18+ with long-term physical and mental health issues to maximize their potential and well-being.
Based within the North East Hants area, the successful candidate will have a minimum of two years’ experience working within community mental health services and possess an ability to work autonomously within a primary care community setting.
The candidate must have a full and valid driving licence and access to a vehicle for work purposes due to the community element of this role.
Interviews are held on Wednesday 16th October
Main duties of the job
Working within Primary Care, you will be able to facilitate mental health/cognitive assessments and formulate diagnosis, treatment/post diagnostic plans with the supervising Consultant Psychiatrist.
The successful candidate will favour a person-centred, psychosocial approach to practice, enabling independence and avoiding hospital admissions where possible.
Excellent risk assessment, communication and documentation are an essential skill set for candidates, along with a willingness to problem solve and signpost to other services, where appropriate.

- Professional Qualification in Mental Health Nursing or Occupational Therapy

- Non-medical prescribing

- Experience of providing brief psychosocial interventions with adult/OA mental health patients
- Experience of conducting MH assessment/Memory assessments
Desirable criteria
- At least two years experience working in a community setting
