KEY RESPONSIBILITIES OF THE POST

Clinical Skills

§  Assess and implement the care needs of children newly-diagnosed with diabetes, and their families.

§  Contribute to the development of programmes for continuing care, developing specialist knowledge and skills within the speciality and use this to influence patient care positively.

§  Make adjustments in insulin therapy within the agreed guidelines in conjunction with the MDT

§  Assess risks to children/families that need to be managed in relation to health and wellbeing and raising any concerns with the MDT.

§  Contribute to the development of clinical standards within the diabetes speciality, working within guidelines set by National Institute of Clinical Excellence (NICE) and the National Service Framework (NSF).

§  Liaise with nursing staff on the ward and the community consultants in other specialities, including safeguarding.

§  Contribute to the seamless transition to adult services.

Management

§  Act as keyworker for a defined caseload with the support of the MDT. Prioritise own workload and manage time effectively when working independently or in collaboration with others.

§  Support the organisation and management of the specialist outpatient clinics for children and young people aged 0-19 years in conjunction with the Paediatric Diabetes MDT.

§  Act as a positive role model to all staff and promoting effective teamwork.

§  Contribute to continual quality improvement and implementation of evidence-based practice and changing own practice accordingly. Including supporting with the implementation and education of new diabetes technology's as and when they are available to CYP and their family's and the wider Paediatric Teams.

§  Takes part in discussions regarding implementation of changes to service.

§  Participate in the annual peer review process.

§  Actively participate in the East of England Paediatric Diabetes Network.

§  Participate in audits and research, both local and national.

§  Maintain up to date knowledge of new products or equipment relating to the speciality.

§  Contribute to the development and implementation of protocols, policies and guidelines relating to the speciality.

Professional Responsibility

§  Exercise professional accountability and uphold the Nursing & Midwifery Council Code of Professional Conduct.

§  Use reflection to formulate and prioritise areas for self-development. Develop and maintain own professional knowledge and skills through life-long learning.

§  Work within regional and national policies and directives and attend appropriate seminars and courses within speciality.

§  Contribute to local and national clinical audit and research initiatives.

§  Act as an advocate on behalf of children/relatives, ensuring the child/relative’s interests, rights and privacy are respected.

§  Be conversant with all policies and procedures, including major accident, resuscitation, fire, health and safety, disciplinary, grievance, industrial relations and safeguarding.

Educational Responsibility

§  Contribute to the development and implementation of structured education programmes for children/families/carers to promote their understanding and empower strategies for self-management.

§  Participate in appropriate support groups.

§  Participate in the training and updating of ward staff and student nurses in the speciality of diabetes and offer clinical support within the sphere of the speciality.

§  Participate in formal teaching with school teachers and ancillary staff within schools, to ensure they are aware of how to manage children’s diabetes at school.

Policy and Service Development

§  Interpret national policy and directives and participate in the development of specific strategies and plans for the specialty and Trust in response to these in conjunction with the MDT.

§  Participate in the implementation of agreed service strategies and plans.

§  Participate in collaborative working relationships and effective communication between all members of the MDT.

§  Maintain effective communication systems within clinical area.

§  Ensure the maintenance of accurate records with regard to legal aspects of confidentiality and management information requirements.

§  Enable results of benchmarking and audit in conjunction with the MDT.

§  Contribute to clinical decisions regarding patient care with patients’ diabetes in conjunction with the MDT.

§  Maintain and continually seek to improve professional knowledge by attending study days, workshops and conferences in addition to professional networking and reflective practice.

§  Comply with and support the Trust’s clinical governance system, risk management and complaint procedures.

Professional/Educational

§  Develop and maintain own professional knowledge and skills through life-long learning.

§  Use reflection to identify and prioritise areas for self-development.

§  Attend annual Trust mandatory training.

Administration

§  Maintain accurate records in line with Trust policy and the NMC records and record keeping guidelines.

§  Ensure all patient contacts are documented and letters are written, currently using E Care
