Job overview

We are looking for a Community Support Worker to join our supportive, friendly and professional Crisis Resolution home treatment team based in Telford. 

Working as part of the team the post-holder will provide support and spend therapeutic time with an allocated group of people with acute mental health needs. This is to promote the persons recovery and to assist them to maintain their place in the community and manage transitions between services. The post holder will also assist the Shift Co-ordinator/ Lead Professional to assess, plan, implement and evaluate individualised care plans. The role will require people to be able to drive; work days, nights and weekends on a rota basis.

Our team also provides a gate keeping service to the in-patient wards at Redwoods in Shrewsbury. To facilitate early discharge and provide an effective and viable alternative to hospital admission for clients in crisis by developing an individualised approach to the practical and functional needs of the service user.

MPFT will offer a welcome bonus of 10% of first year salary- at 50% on commencement of employment and 50% following 12 months employment.

Main duties of the job

You will provide active support in the assessment, development, implementation and evaluation of service user care, responding quickly to those referred with urgent and high-risk mental health needs and provide collaborative, effective, evidence-based interventions at the right time with the right person.

Working in a multi-disciplinary team and in partnership with internal and external agencies, you will:

  • Assist in providing  specialist assessment and care planning in conjunction with service users and carers
  • Help to carry out existing treatment plans and undertake home treatment
  • To promote service-users recovery and to assist them to maintain their place in the community or manage transitions between services.

Essential criteria
  • NVQ 3 in (Mental) Health and Social Care with demonstrable evidence of numeracy and literacy skills (equivalent GCSE’s or Functional Skills)

  • Advanced Apprenticeship in (Mental) Health and Social Care

Experience
