Join our vibrant team and seize the opportunity to be a Specialist Dementia Practitioner in the Dementia Liaison Team at UHNM County Acute Hospital, Stafford. This role is your chance to play a crucial part in transforming the lives of patients with dementia and cognitive issues across all hospital wards.

As a Specialist Dementia Practitioner, your role is dynamic and impactful. Manage the workload, records, and reports of the dementia liaison team, showcasing your expertise acquired through specialised training and experience. Handle a diverse clinical caseload, reporting on key activities and contributing to the achievement of performance indicators.

Collaborate with external agencies, providing outreach assessment and support in the innovative Discharge to assess process. Be a key player in supporting wards to create specialised care programs for patients with dementia, shaping the future of dementia care.

  • Expert Assessment and Care: Take the lead in conducting expert assessments, planning, implementation, and evaluation of care for a specific group of service users. Your expertise will be a valuable resource within the Acute Trust, contributing to a high standard of care.
  • National Standards: Be a key player in ensuring that care aligns with the National Service Framework and NICE Guidelines, championing best practices and elevating the quality of service delivery.
  • Diverse Responsibilities: Enjoy a dynamic role that involves participating in priority-setting procedures, allocating work effectively, and covering all levels of priority within the service. Your contribution will be crucial in maintaining the efficiency of our operations.
  • Clinical Judgement: Utilise your clinical judgement and expertise in screening assessments to determine the appropriateness of referrals. Make recommendations for further action, including medical investigations, mental health assessments, or referrals to other team members.
  • Comprehensive Patient Care: Engage with patients on acute hospital sites or other suitable locations, ensuring comprehensive assessments of needs. Develop action plans for care in collaboration with patients, fostering a patient-centred approach.
  • Mental Health Expertise: Attend ward rounds to provide mental health expertise, contributing to discussions about cases and ensuring the seamless integration of mental health care within the broader healthcare framework.

Your contributions extend to developing protocols and service changes, supporting in dementia education, and ensuring accurate record-keeping aligned with UHNM and MPFT policies. Forge strong collaborations with County Hospital, MPFT, and other providers, contributing to care planning and reporting on governance issues. As a Specialist Practitioner, you'll work in a General Hospital setting, addressing unpredictable work patterns and client needs with resilience and compassion.

Your impact goes beyond the hospital walls. Provide specialised assessments, mental health interventions, consultation, support, and clinical supervision to health professionals within the trust. Your role extends to outreach assessment and support for patients on a discharge-to-assess pathway, fostering partnerships with various services and agencies.

  • • Evidence of clinical expertise in Mental Health Nursing/OT/Social Work – Dementia Care
  • • Experience post qualifying with the client group
  • • Clinical supervision experience / training
