Job overview

We have an exciting opportunity available within our 0-19 Stoke on Trent Integrated Health Visitor and School Nursing Hub.

Our Hub provides a single point of access for children, young people and families accessing our Health Visiting and School Nursing Teams across the City of Stoke on Trent. This service aims are to process, assess and determine the health needs of children and young people (CYP) to ensure their needs are met with the correct service provision through liaison and multi-agency working.

You would be the first point of contact for clients and professionals into the service.  Duties will include making telephone calls, using patient electronic records, liaison with professionals, making and changing appointments.

 

Main duties of the job

  • Answering the telephone as first point of access to the service
  • Offer a word processing service to other teams within the service
  • Maintain excellent channels of communication to all team members
  • Process referrals and determine the appropriate professional to send to following processes and protocols
  • Maintain an effective office support system
  • Input into electronic diaries for clinical and non-clinical activities
  • Take and relay any messages and deal with requests when necessary
  • Undertake any training and development to fulfil the requirements of the post.
  • To use electronic patient records RIO to ensure children and young people are on the system, book and change appointments.
