Job overview

The Community Mental Health Rehabilitation Service is a new and developing service that works with people with complex severe and ensuring mental illness (psychotic illnesses) and their families within their local communities.  

The service will provide specialist assessment, treatment and support to stabilise people’s symptoms and help them gain/regain their skills and confidence to live successfully in the community – adopting a team approach to care delivery.

The service will support individuals in their own homes in the community, in supported accommodation and those returning from out of area hospitals.

The service has strong working relationships with other services both internally and externally, striving to make a significant difference in our communities.

We promote regular CPD opportunities both internally and externally and support individuals to develop specialised nursing skills to work in a complex area of mental health care.

Main duties of the job

  • Undertake clinical assessments to establish the person’s needs, goals and risks and generate a formulation based care plan with all involved.
  • Deliver recovery focussed and evidence based interventions, in accordance with an agreed care plan, to an identified group of people
  • Manage a caseload, as named keyworker, of individuals with complex severe and enduring mental illness
  • Administer medication, complying with relevant Trust procedures and NMC Guidance.
  • Complete and contribute to robust risk assessments and risk management plans - deliver and promote positive risk taking
  • Participate in the development of the service - assist in ensuring that the aims and objectives of the service are fulfilled and to identify factors which may inhibit these from being achieved.
  • Possess excellent communication and interpersonal skills, and have an ability to build positive multidisciplinary team relationships, and work collaboratively with external agencies, patients and their families/carers.
  • Demonstrate good leadership skills

Qualifications

Essential criteria
  • Registered mental health Nurse on NMC Register.
  • Post-registration experience, evidence of formal courses and training at diploma/degree level
Desirable criteria
  • Leadership qualification/course
  • Registered Practice Supervisor

Experience

Essential criteria
  • Evidence of post registration experience working with adults experiencing complex severe and enduring mental health needs
  • Experience of working with people who have difficulty in engaging with services and have complex mental health needs
  • Experience of working in community setting and making decisions autonomously
  • Experience of assessing, planning, co-ordinating and managing people with complex severe and enduring mental health problems in a community setting.
  • Experience of integrating care delivery across multi-disciplinary /multi agency teams

Skills

Essential criteria
  • Knowledge of the Mental Health NICE Guidelines and Long Term Plan
  • Understanding of the principles of the Care Review Process and Community Care Act
  • Knowledge of NMC Code of Conduct
  • Skills of assessing, planning and evaluating care, for people with mental health related difficulties, utilising the principles of Recovery
  • Skilled in the assessment and management of clinical risk and developing and communicating appropriate risk management plans
  • Knowledge of the physical health needs and monitoring requirements for people with severe and enduring mental illness
  • Skilled in undertaking and interpreting physical health investigations and providing physical health promotion advice including: weight, BMI, blood pressure, smoking, alcohol, drugs (prescribed and non-prescribed
  • Skilled in the administration of pharmacological interventions
  • Range of Interpersonal skills, (communication, listening, working collaboratively) maintaining good working relationships with service users/carers, other service providers and agencies
Desirable criteria
  • Skilled in utilising specific assessments and outcome measures including; GAD7, PHQ9, CAARMS, PANNS, SWEMWBS, Lunsers, GASS
