Practitioner – Liaison and Diversion Pathway

This is an exciting opportunity for a Band 6 Practitioner role to work as part of the expanding Health & Justice Service in West Mercia (Herefordshire).

The post will be based in Hereford.

This post will increase the capacity of the existing L&D team and will be working closely with the current staff.

This role will include providing high quality assessments for people who are in the criminal justice system. The service will be delivered to peoples who present with any vulnerability including but not limited to; mental health, physical health, learning disability, head injury, unemployment, housing, educational needs etc.

The successful applicants must be experienced in risk assessment, highly motivated and someone who is looking for a new challenge and have the skills to make a difference.

Due to the nature of the work posts will be subject to police vetting.

Travel between criminal justice settings in West Mercia (Herefordshire, Worcestershire and Shropshire) may be required.

  • Delivering a high-quality clinical service to patients and their families.
  • Managing complex assessment in courts and the community.
  • Responsible for clinical and caseload supervision of other team members where identified.
  • Diverting and liaising with multiple criminal, police courts and other partners.
  • To provide written reports for Crown or Magistrates Courts or to attend and give verbal evidence as required. To offer considered recommendations to these bodies.
  • Responsible for participating in the setting of quality standards, including the auditing, monitoring, and reviewing of practice in line with current clinical guidance practice and policy.
  • Assess patient's individual care needs, agree, and refer on to appropriate agency.
  • Complete and contribute, where appropriate to robust risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence.
  • Maintain an effective reporting system by observing and reporting verbally and in writing on patient condition
  • Utilise agreed outcome measures to review the effectiveness of process, as part of Liaison and Diversion Service.

The Post holder will be expected to conduct assessments within the Community, Custody and Court setting, providing relevant information to partner agencies, Court & Probation services to support people in custody and the community, offering diversion options from the criminal justice system to more appropriate services where needs are identified.

The post holder will be required to work autonomously within this role and be accountable for managing referrals; prioritizing referrals and ensuring clinical and electronic record keeping standards are adhered too.

Staff would need to be mindful and skilled to work with people of different ethnicities and backgrounds. They must also be able to demonstrate experience of working with both adults and children/youths.

QUALIFICATIONS & TRAINING

  • Registered Mental Health Nurse / Learning Disability Nurse on NMC Register.
  • Registered Social Worker.
  • Post-registration experience, evidence of formal courses and training at diploma/degree level.

  • Leadership qualification
  • BSc degree in nursing or equivalent level
  • Qualification in providing supervision Qualification in coaching

Experience

  • Evidence of extensive post registration experience with working with Clients experiencing mental health related difficulties
  • Experience of working with Clients who have difficulty in engaging with services and have complex mental health needs
  • Experience of working in community setting and making decisions autonomously

SKILLS, KNOWLEDGE & ABILITIES

  • Knowledge of NMC Code of Conduct
  • Understanding of the principles of the Care Programme Approach and Community Care Act
