Following a successful launch of the Band 5 Care Coordinator role within our workforce last year, we are looking to recruit an additional 3 full time, permanent Band 5 Care Coordinators across Sheffield Community CAMHS.

The post-holders will work within Sheffield Community CAMHS as part of a multi-disciplinary team delivering care coordination to young people and their families to enhance access and collaboration throughout their patient journey in CAMHS alongside the wider multi-agency network.
You will work as a valued member of the team under the supervision of Registered Nurses and working alongside other multi-disciplinary professionals to develop the specialist skills and knowledge required to work in a child and mental health setting and enable you to provide high-quality, goal-based interventions using a range of low-level psychological interventions.
You will act as lead link to young people and their families, and to effectively respond to telephone enquiries in relation to aspects of their care, by liaising with relevant colleagues. In addition, you will develop and maintain effective liaison and two-way information sharing between the multi-agency network, assessing, and integrating issues relating to safeguarding, education, and transitions, into the overall therapeutic process.
Our team work closely together, enjoying the ability to share expertise and resources to deliver a first-class, high-quality service. To do this, we are making improvements to our service to ensure there is the investment in our delivery model and staff by offering excellent supervision and training to deliver the service we want for children and young people attending mental health services.

- Evidence of ongoing professional development

- Previous experience of working with children and young people or Adult mental health experience

- Knowledge of the principles of clinical governance and its application
