THIS POST IS AVAILABLE TO ALL APPLICANTS WHO CAN DEMONSTRATE RELEVANT PROFESSIONAL REGISTRATION, EDUCATION TO DEGREE LEVEL AND ABILITY TO DEMONSTRATE EQUIVALENT LEVEL OF KNOWLEDGE

The post requires an appropriate Professional Registration (NMC, HCPC etc) , a strong proven clinical knowledge within mental health and evidence of ongoing professional development. Previous experience of working with adults across the age range is essential.

The Hospital Liaison Team provide specialist mental health assessment for those patients presenting at the acute hospital with various physical health issues alongside mental health concerns.

The Team work in line with PLAN (Psychiatric Liaison Accreditation Network) standards and Referral to Assessment targets. 

The role requires an ability to work under pressure maintaining good working relationships with acute colleagues and high standards of care.

The role involves the use of IT systems across both the mental health and acute trusts, so proficient use of IT is necessary.

The provision of  a  specialist responsive, needs-led mental health assessment and formulation for individuals presenting to the acute hospital who experience a range of mental health difficulties.

Referrals originate from a wide range of sources  within the acute hospital environment and high quality interdisciplinary communication is essential.

There is a teaching, education and support element to the role, assisting non-mental health staff to respond to mental health needs more effectively in a range of environments.

The Team are aligned to the Psychiatric Liaison Accreditation Network (PLAN).  All Assessments and interventions are in line with the PLAN standards which high quality psychosocial assessment and care. Assessments are undertaken in collaboration with service users their families and health and social care services. 

The team facilitate placements for students from a range of health professional backgrounds, therefore, the role entails a Practice Educator function.

1. Liaison and consultation
1.1 Be responsible for the full assessment of patients presenting with 
Dementia and a range of undifferentiated mental health and psychological 
problems. As a result of this assessment, you will be responsible for the 
development, implementation and evaluation of programmes of care, 
without direct supervision.
1.2 Provide a comprehensive consultation-liaison service to clinical teams, 
assisting them to formulate appropriate plans of care and make decisions 
in cases where patients may be exhibiting complex needs and/or 
challenging behaviours.
1.3 Contribute to effective case management for individual patients and 
carers, including facilitating meetings, liaising and/or negotiating with other staff.
1.4 Promote a positive attitude regarding mental health across the general 
hospital and residential care home settings.

2. Clinical practice
2.1 Utilise a range of clinical skills appropriate to the individual patient's needs and the clinical setting.
2.2 Undertake specialist psychosocial assessments with patients presenting with complex physical and mental health needs; highly developed risk 
assessment and management skills are required in order to undertake this 
function effectively.
2.3 Prompt referral on to secondary mental health services, social services 
and third sector organisations as appropriate.
2.4 Flexible, needs-based follow-up of individuals until ongoing care 
arrangements are agreed and in place.
2.5 Concise, effective liaison and feedback to clinical staff within the various settings regarding advice on appropriate management strategies for individuals referred.
2.6 Effective liaison and communication with patients, relatives, carers, 
primary health care and secondary mental health services.
2.8 Role model clinical excellence and high standards of nursing practice to 
other staff.
2.9 Initiate, participate in and oversee effective implementation and 
administration of the Mental Health Act (1983), and the Mental Capacity Act (2005), including the use of Deprivation of Liberty Safeguards and Best 
Interest Decision making process.

3. Communication
3.1 Demonstrate the ability to receive and give complex clinical information clearly and effectively in a timely manner.
3.2 Demonstrate the ability to communicate clearly and sensitively in 
situations that may be hostile and/or contentious.
3.3 Demonstrate a high level of interpersonal skills, self-awareness and 
empathy in communications and interactions with patients, carers and other staff.

4. Management
4.1 Following negotiation, take responsibility for agreed clinical and 
managerial projects associated with the provision of the service. 
4.2 Ensure the promotion of the health and safety, well-being and interests of patients, staff and visitors that come into contact with the service and its 
environs.
4.3 At all times adhere to the NMC Code of Professional Conduct.
4.4 Supervise, monitor and appraise the performance of registered and 
unregistered staff within sphere of responsibility.
4.5 Take charge of the clinical service in the absence of the person who has 
continuing responsibility.

5. Audit and evaluation
5.1 Participate in the delivery of appropriate audit, evaluation projects and 
service development (including data collection where appropriate).

6. Education
6.1 Actively participate in the identification of own learning and educational needs and demonstrate achievement against personal development plans 
(PDPs).
6.2 Participate in the mentoring and education of a range of students on 
placement.
6.3 Act as a mentor and preceptor, when appropriate.
6.4 Participate in the planning, development and delivery of multi-professional education programmes and training sessions for a range of clinical and managerial staff.
6.5 Provide clinical supervision to colleagues and students, as required.

7. Quality and Effectiveness
7.1 Promote and encourage user and carer feedback regarding the service.
7.2 Participate in and undertake appraisal and clinical supervision.
7.3 Adhere to relevant trust health and safety and clinical policies.
