Job overview
Are you looking for a new challenge as a Medical Nurse Practitioner making a real difference for older people living with Frailty? We are recruiting for an enthusiastic and motivated B7 MNP post to work within the supportive and forward thinking team at the RUH.
You will work closely with consultants, ACPs, nursing and therapy colleagues in ED and the front door, delivering multi-disciplinary, patient-centred care, advising on discharge or admission with the complex caseload this interesting group provides. You will be supported with teaching and development skills and opportunities to build your own knowledge and learning and shape your future pathways in this area.
Main duties of the job
-
To hold responsibility for own case load and provide a high standard of assessment, diagnosis and planning for:
- Frailty - a dynamic team of geriatricians, medical nurse practitioners and occupational therapists and physiotherapists providing early assessment and intervention in the emergency department and Older Person’s Rapid Assessment Area (OPRAA) to those patients living with frailty.
- The Older Person’s Assessment Unit (OPAU) and Older Person’s Unit Short Stay (OPUSS) which are our short-stay, older people’s inpatient ward for our patients who require a bed for less than 72 hours.
- Provide clinical support, teaching and education to colleagues and families
- Support with future planning and management of Frail patients
- Represent MNPs and work with other members of MDT to support service improvement and best practice.

Detailed job description and main responsibilities
The post holder will be an experienced, compassionate and courageous nurse who demonstrates critical thinking and who upholds core nursing values.
The post holder must clearly demonstrate well-developed specialist knowledge and skills at an advanced level (Degree/Masters) in a specialist area of practice or with a defined patient group through recognised accredited training programmes.
The post holder will be expected to provide support to the Medical and Nursing teams in the provision of professional, clinical and managerial support, advice and leadership, ensuring that high levels of patient care is maintained in line with professional and statutory guidelines.
The post holder will be required to undertake extended roles in line with patient needs and professional training.
The post holder will where appropriate, provided initial assessment to patients and working at an advanced level preforming clinical procedures and tasks that have been previously undertaken by medical and/or technical staff.
The post holder will practice autonomously and also within a defined team/multi-disciplinary team and in accordance with local guidelines and protocols to ensure patient safety needs are paramount.
The post holder will formulate clinical decisions and treatment plans within local guidelines/protocols to manage acute and chronic illness and respond to actual and potential health problems appropriately.
The post holder will be grounded in nursing and will undertake advanced clinical practice procedures or tasks. The post holder will also provide holistic nursing care or therapeutic nursing interventions to ensure patient safety is maintained.
The post holder will work according to the NMC code of Professional Conduct and NMC standards of conduct, performance and ethics for nurses and relevant professional guidelines and carry continuing responsibility for the assessment of care and health education needs through-
o The development, implementation and evaluation of programmes of care
o The provision of professional leadership through clinical expertise to junior staff
o To continue to develop clinical and management skills and experience
o The review of nursing and medical practice and implementing appropriate changes based on research/best practice within clinical environment
o The development and implementation of Audit, Quality and Risk management initiatives.
MAIN DUTIES & RESPONSIBILITIES:
Clinical and Educational
· Recognise and work in line with professional code of conduct, local and national guidelines in order to deliver safe and effective evidenced-based care.
· Provide the first point of contact for all ward staff within the agreed speciality to autonomously assess, diagnose, plan and implement treatment/interventions for patients changing conditions
· Assess, diagnose. Plan, implement and evaluate interventions/treatment for patients with complex needs
· Attend and assist with all types of emergencies on site utilising skills and experience as required
· Liaise with and refer patients to other members of the health care team/multi-disciplinary team, primary care or intermediate care as appropriate
· Provide clinical support to all staff and ensure patients safety
· Ensure clinical skills are maintained and updated to provide consistent and quality care
· To participate in the development of guidelines and protocols, recognising the need for any further role extension to provide vision and innovation regarding new developments and guidelines to practice.
· Prescribe and review medications for therapeutic effectiveness (upon successful completion of NMP course), appropriate to patient needs and in accordance with evidence-based practice and national and local protocols
· Be familiar with Clinical; Governance and its requirements as well as trust guidelines and policies
· When required, will consider making safeguarding and DOLS referrals following discussion with medical and nursing staff
· Disseminate learning and information gained to other team members in order to share good practice and inform others about current and future developments (e.g., courses and conferences)
· Assess own learning needs and undertake learning as appropriate
· Provide an educational role to patients, carers, families and colleagues in an environment that facilitates learning
Professional Responsibilities
· To develop and maintain high standards of care, ensuring safe practice and at all times to work within the NMC code of Professional Conduct and NMC standards of conduct, performance and ethics for nurses

Managerial Responsibilities
· Attend and assist in all non-clinical emergencies including Fire, Untoward/Adverse Incidents, breaches of security and Major Incidents.
· Be pro-active when dealing with potential complainants, resolving issues where possible before they become formal complaints. However, where necessary refer on to appropriate persons in line with Trust guidelines.
· Be aware of clinical risk management, COSH guidelines and report any concerns to appropriate persons
· Ensure effective use of available resources
· To represent the Nurse Practitioner team at appropriate meetings
· Where possible contribute to managerial and developmental meetings on the unit, such as morbidity and mortality, clinical governance and journal club
· To maintain communication networks across the trust and raise the profile of the Nurse Practitioner team to all staff within the hospital
· Promote and encourage innovative nursing practice in order to improve patient centred care
· Liaise with other team members to ensure effective usage of beds within the speciality
· Support the management of outlying patients; ensuring where possible patients are placed in appropriate speciality
· Is accountable for own professional actions and works autonomously
· Communicate problems in bed management where appropriate and support the active and safe discharge of patients

Qualifications and Training
Essential criteria
- Registered Nurse Part 1, 1,12
- Registered with the Nursing and Midwifery Council
- Relevant clinical supervision training and experience
- Registered Non-Medical Prescriber

- Minimum one years' experience as a Nurse Practitioner at Band 7
