Discharge Liaison Nurse / AHP

Supporting and facilitating supported, complex or delayed discharges/transfers from the Royal United Hospital, Bath.
Working as part of a multi-disciplinary team, developing good communication networks and joint working practices across the RUH, community hospitals, community services, Integrated Care Boards and social services.
This vacancy is open to RGN/ OT / PT and Social work colleagues.
Prif ddyletswyddau'r swydd
To maintain effective communication and promote professional, caring partnerships by acting as a role model in accordance with the Trust values
Provide support for ward staff in managing those patients with supported and complex discharge needs
To participate in multi-disciplinary ward meetings/ward rounds with medical staff as required for those patients with complex needs
To provide expert advice and support for ward staff and hospital multi-disciplinary teams in order to facilitate effective discharges and transfers
Advise ward staff on the completion and maintenance of an ongoing discharge care plan and ensure records are concurrent and the plans are evaluated by ward staff
To manage complex and sensitive situations, ensuring patient confidentiality at all times
