A new role has become available within The BANES Recovery Team.
We are a friendly and supportive team who are passionate about improving the lives of service users with complex mental health needs. As a care co-ordinator in the team you will be responsible for a caseload of service users with a range of complex mental health needs, working within the CPA framework to deliver a range of health and social care interventions.
We strongly encourage personal and professional development - and once you are established within the team, there would be opportunities to undertake training to support you within the role, from example, SCM, DBT, MBT or understanding psychosis.

We are looking for any profession with a registration to apply, including Social Work, Clinical Associates in Psychology, Occupational Therapists and Registered Mental Health Nurses.
Prif ddyletswyddau'r swydd
Provide a Mental Health service to an allocated caseload of Service Users, and deliver a set of defined interventions within that caseload, having responsibility for the assessment of needs, the planning and implementation of treatment and care to meet those needs as agreed with multidisciplinary team and/or supervisor, coordinating care (with supervision) for particular Service Users, and contributing to the evaluation of care across a variety of settings where Mental Health or Specialist Treatment is available.
Liaise as appropriate with other members of the multidisciplinary team and other agencies involved in the care of individual service users.
Participate in Integrated Governance processes, particularly as they relate to own practice, contributing to the monitoring of service provision and performance, and standards of care, through data collection and audit processes.
Liaise with other services and agencies involved in provision of services to people with Mental Health and other vulnerabilities.

- Essential Demonstrates a good understanding of recovery principles and the role of secondary mental health services in the delivery of care. Is able to contribute to the delivery of a range of possible key therapeutic interventions. Demonstrates significant experience of, and relevant professional practice. Demonstrates significant experience of assessing risk and developing risk management strategies. Able to articulate a sound understanding of the relevant legal frameworks/legislation including CPA process, Mental Health Act , Mental Capacity Act, and Safeguarding. Well developed verbal communication skills, able to engage effectively with people at all levels including when a more assertive approach is needed. Well developed active listening skills, which allow for reframing and testing of understanding. Well developed written communication skills, with experience of compiling and sorting notes and reports, establishing statistical data and analysing information.
