Community Healthcare Support Worker

In addition, applications to the Health and Care Visa route for those occupations paid by NHS bands that are on the immigration salary, a list of skilled jobs which have lower salary requirements, will need to meet the national pay scale for their role or the new minimum salary threshold of £23,200 (whichever is higher). We advise that you check that the role you wish to apply for meets the requirement for sponsorship before applying.

An exciting opportunity has arisen for highly motivated individual to join our Community Team within South East Shropshire in the provision of Planned and Admission Avoidance care to patients in the community.

We are looking for a caring, motivated and friendly individual to join our busy, professional and dynamic team. You should possess commitment to interdisciplinary team working and innovative delivery of excellent standards of care. 

The post holder will assist and support other members of the team in giving care / undertaking phlebotomy to patients within a designated area, under the supervision of a qualified nurse.

The opportunity to develop additional skills within the HCSW job description and support in career progression through accessing formal training and clinical competence.

Prif ddyletswyddau'r swydd

  • Promotes a ‘can do’ attitude and determination to develop and sustain a dynamic and responsive clinical and administrative support service within a Community Hospital / Community Team setting which delivers best practice.
  • To deal appropriately with interruptions, unpredictable events and behaviours. 
  • To organise and deliver skilled clinical support work / phlebotomy without direct supervision. 
  • Gain valid informed consent and work within legal frameworks with patients. 
  • To liaise with nursing and therapists with regard to patient care, highlighting any variations to expected outcomes from agreed treatment programmes.
  • To provide basic verbal and written clinical reports to the supervising Nurse and / or therapist and to document patient interventions/findings in patient records in line with Trust Record Keeping Policy.

Qualifications

  • GCSE Level C+ in English and Maths or Level 2 functional skills in English and Maths or willingness to work towards this.
  • Level 2 in health and social care (or equivalent) or willingness to work towards this.
  • Care Certificate or willingness to undertake

Experience

  • Awareness of accountability issues e.g., confidentiality.
  • Awareness of the role of the Healthcare Support Worker.

Knowledge

  • Knowledge of the role of the health care assistant role in context, including phlebotomy
  • Knowledge of situations and circumstances that can impact on patient safety and demonstrate understanding of how the healthcare support worker contributes to maintaining patient safety
  • Good interpersonal skills (written and oral)
  • The ability to communicate effectively with patients and staff both written and orally and demonstrate a level of English language and grammar commensurate with writing in service user case notes.
  • Ability to remain calm when faced with a potentially difficult situation
