Sylwer bod ychwanegiad dros dro ar gyfer Bandiau 1,2 a 3 i adlewyrchu ymgorffori'r ychwanegiad i’r cyflog byw, sef £12 yr awr - £23,465 y flwyddyn. Bydd yr ychwanegiad dros dro hwn yn weithredol hyd nes y bydd y codiad cyflog blynyddol ar gyfer 2024/25 wedi’i gadarnhau

Trosolwg o'r swydd

The post holder will provide knowledge, skills and experience in the field of respiratory practice and act as a resource to advise, educate and support the care of people with long term respiratory disease.

The post holder will provide specialist complex assessment, advice and education via Community Hospital specialist led clinics and on home visits to patients requiring respiratory and/or oxygen assessment.

This post holder will be accountable to the Health Board via the Line Manager and Clinical Lead to provide safe, high quality care to patients by implementation of policies/ standards and guidance produced both nationally and locally, these will include from NMC, HCPC Welsh Government, Powys Teaching Health Board, and others.

Prif ddyletswyddau'r swydd

To work collaboratively within the Multi-Disciplinary Team and with other health care professionals to ensure a consistently high quality and equitable service throughout the Health Board.

To act in an advisory capacity, using clinical knowledge to inform decision 
making regarding patient care. Support local initiatives at removing interagency barriers and maintain excellent professional links with health and social care providers to promote effective communication and joint working practices.

To work autonomously, making clinical decisions within own competency and ensuring effective caseload management continuously reprioritising to meet the changing clinical demands of the service. Take appropriate action to ensure any clinical risks are identified and minimised.

Be responsible and accountable for an identified selected case load, working with the patient and the MDT team to reduce length of stay and co-ordinate care across all settings of the care pathway to facilitate discharge.

To support the Team Lead in the efficient use of resources within the budgetary allocation, including authorising expenditure.

The person appointed will be expected to undertake a range of duties to include:

To work within and adhere to the Professional code of conduct; standards and guidance at all times, ensuring that issues of concern are escalated appropriately. To act at all times as a role model, presenting a professional approach and ensuring the standards for professionalism are set and displayed within the team. To provide professional advice to where applicable within the team. 

To work autonomously, making clinical decisions within own competency and ensuring effective caseload management continuously reprioritizing to meet the changing clinical demands of the service.

Qualifications and/or knowledge

  • Live registration with the NMC as Registered Nurse or Health Care Professional
  • Degree in Nursing or health related subject or relevant experience
  • Specialist post graduate respiratory qualification or equivalent experience
  • Annex 21 Undertake a development programme to achieve the relevant knowledge, skills, and experience as within the Job Description and Person Specification for this role. Timeframe to be decided upon appointment
  • Experience of proactive caseload management of wide range of patients including those with chronic and respiratory conditions
  • Significant clinical experience in assessing, planning, implementing and evaluating care for the patient group of the caseload
  • Knowledge of current Nursing / Therapy issues within NHS care systems
  • High level of knowledge of NMC code of conduct/HCPC and professional standards/guidance
  • Ability to use computer systems to a welldeveloped level (e-mail, word processing, spreadsheets)
  • Well-developed verbal and written communication skills
  • Effective multi-agency team player, including the ability to engage positively with carers
  • Able to deal with difficult negotiations across care boundaries
  • Demonstrable ability and track record of acting with care, compassion, promoting dignity and respect, role modelling the highest standard of professionalism
  • Ability to motive, enthuse and encourage innovation within the team through effective communication skills
  • Knowledge and skills to manage patients with complex needs
