Would you like to join a growing team and be part of transforming services for those who have mental health rehabilitation needs and being part of the development of the new service from the very beginning? If you are a senior mental health clinician with experience of working with people with serious mental health needs and keeps service users and their loved ones at the heart of everything they do we want to hear from you.
As a result of the extra investment that is coming into Mental health services as part of the NHS Long term Plan, we are excited to be able to expand and transform what we offer to this service user group and their families. You will work as part of a community rehabilitation team as part of the wider rehab pathway offering interventions that are in line with National guidance.
The post holder is identified as having specialist knowledge in the mental health needs of adults and their families with specific focus and remit to deliver significant improvements for patients that have long term mental health conditions and rehabilitation needs.
Prif ddyletswyddau'r swydd
To provide the Community Rehabilitation Team (CRT) with the defined level of expertise as a registered Mental Health Nurse.
This includes providing a specific expertise in assessment and care planning for service users within the CRT service. Advocate rights of individuals to live independently, with their families, with careers or with placements. Continue to work with people with mental health needs by working with the service users in a holistic and person-centred approach.
To provide a lead role as a Mental Health Nurse when working with fellow colleagues within the community rehabilitation team. Assist the team in assessments and formulations, care/management/crisis and placements, this includes inpatient units within the trust and wider placements within the local authority. Taking a particular lead in medication support/management or those service users under national legislation of the Mental Health Act. aswell as and placement plans to enable to CRT to assist service users transition from in-patient to discharge back into the community.
To maintain a caseload of service users within the CRT, attend group formulation/planning meetings and support fellow colleagues with formulation. Undertaking activities with service users in the community.
To provide realistic, timely and occasionally intensive Nurse led interventions to patients within a community setting.
The role will be working with service users with a serious or complex mental health illness.

This post is eligible for a Golden Hello payment of £2,750 for new starters who are external to the Trust. This is payable in 3 instalments over a 2 year period. This payment is only eligible to new starters who do not already hold a contract of employment with the Trust and have not been an employee of the Trust in the previous 3 years. This post also includes paid enhancements for unsocial hours working during night, weekends and bank holidays

- Core Professional qualification to degree / diploma level in Nursing or Occupational Therapy.
- Evidence of continued professional development
- Nurse registrants: Completion of Preceptorship.

Experience

- Experience of working with people with complex mental health presentations such as those with psychosis and/ or personality disorders in community settings.
- Knowledge and experience of working under the Care Programme Approach.
- Experience of using psychological formulations to identify intervention targets.
- Experience in the assessment and management of clinical risk.
- Experience of multidisciplinary and multi-agency working

- Experience of working in an EIP service.
- Experience of specialist EIP assessments and interventions
- Experience of working therapeutically with individuals experiencing psychosis
Knowledge and Skills

- Good working knowledge of risk assessment, risk formulation and risk management.
- Able to demonstrate commitment to high quality care for people with psychosis.
- Competent IT skills including word, outlook, excel and power point.

- Excellent analytical and decision-making skills.
- Advanced interpersonal skills demonstrating the ability to communicate and work with a wide range of professionals.
- Ability to work effectively under pressure
- Ability to produce and present clear letters / reports.
- Specialist knowledge of psycho-social interventions and their appropriate use.
