Job overview

The National Rehabilitation Centre (NRC) will transform the future of rehabilitation for people who have experienced injury or illness. In preparation for opening of the new centre in summer 2025, the Rehabilitation Directorate Team will support intensive MSK rehabilitation for a group of patients located on Edward 2 ward at Nottingham City Hospital. Joining the Edward 2 team and the National ambition for rehabilitation, you will have the opportunity to make a positive impact on many people’s lives through being part of their rehabilitation journey and influencing the specialism by pushing the boundaries of rehabilitation forwards with involvement in research and technology. This fixed term Discharge Co-ordinator position in Rehabilitation will be a crucial part of the team. This interim role will be in place as a 0.5WTE position based on Edward 2 until summer 2025. We would like to hear from candidates interested in fixed term or secondment opportunities who believe they can fulfil the attached job description and person specification.

Main duties of the job

The successful candidate will be based on ward Edward 2 at City Hospital.

This person is expected to work in the ward environment, alongside the multidisciplinary team to ensure that effective disacharge planning and communication are managed in order to provide a smooth, timely discharge from hospital.

The discharge coordinator will be working alongside the large team of nurses, doctors, physiotherapists, care support workers and occupational therapists. The role of the discharge coordinator will require working closely with the integrated discharge team, following the discharge to assess pathways as we ll as working with the nursing and medical treams to ensure prompt discharges from the ward via the discharge lounge as much as possible.

Keeping the ward environment clean and tidy will be another key responsibility of the discharge coordinator and they will be expected to be a friendly and approachable member of staff. Other responsibilities will include answering the telephone, liaising with family members and chasing any delays to discharge.

  • Evidence of a good standard of written and spoken English

Experiance

  • At least 2 years of telephone/receptionist/clerical. experience is required

  • Ability to write reports on discharge data and to communicate this information to all levels of clinical staff
