Due to expansion, an exciting opportunity to work as a nurse practitioner with the Hospital24 out of hours team. 

You will be required to provide strong clinical leadership in supporting safe out of hours working and care. 

You will lead a dynamic team on a shift-by-shift basis providing, senior advice and support to junior medical and nursing staff, guidance and education and act as an exceptional role model.

The post holder will be a registered nurse with 5 years demonstrable post registration experience, that includes the ability to work in both a clinical and leadership capacity.

The post holder will have the Independent Nurse Prescriber and Advanced Life Support qualification with proven experience of practice.

The post holder will deliver autonomous clinical care, leadership, support and advice to doctors, nurses and other allied healthcare professionals working in the Trust.

The post holder will have full oversight of the campus, with focus on all adult inpatients that fall under the specified service provision.

The Department has operational responsibility for Hospital24 across the organisation. As such the post holder will be required to rotate between both main campuses that make up Nottingham University Hospitals NHS Trust.

As an experienced Registered Nurse, you will be required to provide strong clinical leadership within a multi-professional team as part of the Hospital24. This role is to maintain the provision of care ‘out of hours.

Hospital24 (formally Hospital@Night) is undergoing service development and review. It is part of the Urgent and Emergency Care directorate that sits in the medicine division.

Recently the service has moved to a 24/7 service provision model and has been given investment to increase the nursing establishment and senior leadership team.

The team is well developed and a great place to work with the opportunity to get career progression.

The service works closely with our medical and surgical colleagues and has a fast paced and positively challenged feel.

Clinical

  • Works autonomously under the remote supervision of the Hosptial24 Consultant Heads of Service to undertake sound clinical decision making and clinical assessments of patients, formulating plans in conjunction with the patient, their family, if appropriate, and the patients' nurse.
  • To take histories, undertake physical examination and order appropriate investigations.
  • To interpret investigations, diagnose and plan treatment and interventions for this group of patients.
  • Works as part of the multidisciplinary Hospital24 team to provide safe and effective clinical care, referring to other specialties as required.
  • Works as part of the Acute Rescue Team (ART) to deliver emergency care to deteriorating patients, including patient transfer to higher dependency areas.
  • Undertakes non-medical prescribing as per the Hospital24 prescribing formulary and NUH guidelines.
  • Works within scope of practice and knows when to refer patients to medical or specialist teams in seeking advice or support.
  • Attends and assists with all medical emergencies utilising Advanced Life Support Skills. Leads the cardiac arrest when required.
  • Supports the wider hospital team to deliver specialist clinical interventions as required.
  • Maintains accurate and legible records of all clinical documentation with regards to Statutory and Trust policies.
  • Undertake clinical activities working autonomously and within the boundaries of their competence.
  • Undertakes Working in New Way (WINW) and development packages as required by the role.

Management

  • Line manage the Hospital24 service in the absence of the Hospital24 Matron or service leads.
  • Lead the shift and be responsible for ensuring the delivery of safe and effective care by the Hospital24 team.
  • Work alongside Medical HR to ensure an adequate provision of service.
  • To be involved in the resolution of complaints at a local and Trust wide level under the supervision of the speciality Matron.
  • Participate in departmental meetings, operational link roles and quality improvement groups as appropriate for role.
  • Undertake any other duties that may reasonably be required.

Wellbeing

  • To provide leadership for the clinical area and to create an environment that is supportive, innovative, challenging and stimulating for all staff.
  • In the absence of ward management teams, be a visible senior nurse to provide management support and wellbeing support to all areas.
  • To act as a role model, maintaining a highly visible profile and have regular contact with clinical staff, patients and their families promoting and demonstrating the standards of care and behaviour expected and act as a resource/advisor to all staff in order to maximise opportunities to improve nursing care and the environment in which it is provided.
  • Promote a safe and caring environment for the team, leading on handovers and SIT reps.

Education

  • To be a reflective practitioner, to remain fully conversant with current clinical practice in your area and to refresh clinical skills as appropriate.
  • To demonstrate continuous learning and development at appraisal by maintaining a portfolio of evidence that reflects the maintenance and on-going development of clinical skills.
  • To attend regular meetings with the Consultant Heads of Service for clinical guidance and support and ensure that development needs are identified and met.
  • To undertake continued, regular, mandatory education within the role to maintain competence, expand knowledge and skill.
  • In conjunction with the Individual Performance Review Process (IPR), undertake individual annual reviews and ensure adequate training needs are met.
  • Contributes to the teaching, development and supervision of all grades of staff including Student Nurses, Registered and Unregistered Nursing Staff and Junior Doctors.

Research / Audit

  • To promote evidence-based practice through the interpretation and dissemination of relevant research findings including the initiation of audit / research project, with the support of a senior clinician.
  • To ensure the dissemination of research through a variety of media, including conferences and presentations, with the support of your mentor and senior staff.
  • In collaboration with other areas and the Clinical Lead/Matron, improve and maintain standards of fundamental nursing care, using tools such as Clinical Benchmarking.

Use of Resources

  • Utilise your agreed establishment effectively to ensure that an appropriate skill mix of staff is available to meet the needs of the service. Ensure that measures are in place to assist in identifying and addressing deviations to agreed levels.
  • Contribute to the management of the recruitment and selection process for staff in collaboration with the Matron/Clinical Lead, as appropriate.
  • Actively participate in initiatives needed for quality improvements, contributing to the work of Trust wide committees and project groups.
