Under the supervision of a nurse you will participate in providing Short Breaks for clients with complex disabilities through a health care model. Working within the requirements of the policies and guidelines of the service, ensure that the clients are supported and assisted with all aspects of daily living, meeting their identified health needs and facilitating an individualised approach to leisure and activities.

You will be required to be a well-motivated individual who possesses good communication skills, is willing to learn and develop the skills and knowledge to support people with learning disabilities and complex health needs.

Duties will include taking part in day-to-day activities and household tasks.  Working within the requirements of the policies and guidelines of the service, ensure that the clients are supported and assisted with all aspects of daily living, meeting their identified health needs and facilitating an individualised approach to leisure and activities. The post holder will be expected to work within the limits of their own competence, responsibility and accountability.

Flexible rostering is offered and you will receive ongoing training, support and opportunities to develop your skills. The service operates 24/7, staff therefore work the full range of shift patterns including days, weekends, night shifts and Bank Holidays.

1: Communication

•     Communicates with all members of the Short Breaks Service, clients and their families/carers on a daily basis in a polite and courteous manner, which is consistent with relevant legislation, policies and procedures. 

•     Utilises communication skills in order to improve effectiveness of communication and to constructively manage barriers.

•     Keeps accurate and complete records consistent with legislation, which may be written or electronic.

•     To support the development of records and assessments promoting the formulation of treatment approaches.

•     To attend and contribute at meetings as deemed relevant.

 

2: Personal Development

•     Takes an active part in own personal development, identifying learning opportunities in order to maintain and further develop knowledge and skills. i.e. network with other service providers.

•     To receive and actively participate in Managerial and clinical based supervision.

•     Actively contribute to the appraisal process through self-evaluation and reflective practice.

•     Works within the limits of own competence responsibility and accountability.

 

3: Health, Safety and Security

•     Takes responsibility for promoting and maintaining a healthy, safe and secure working environment in accordance with legislation, policies and procedures.

•     When working within external services, familiarise self with Health, Safety and Security information (clinical and non clinical)

•     Reports any issues (clinical and environmental) at work that may put the health, safety and security of staff or others at risk.

•     Conversant and comply with the Departmental Health and Safety Policy.

•     To recognise, report and record any untoward incidents in order to maintain the safety of the work environment.

 

4: Service Improvement

•     Offers suggestions for improving services and makes changes in own practice following discussion with line manager/team.

•     Participates in service evaluation and audit.

•     Participates in service evaluation and audit and following analysis, implement required improvements/changes in practice through a team approach.

•     Contribute at meetings as deemed relevant, reporting any areas of deficit in both patient care and service provision.

•     To actively seek feedback/satisfaction, relating to service delivery.

 

 

5: Quality

•     To work within the guidelines of the Short Breaks Service mission statement, operational policy and philosophy.

•     Recognise and aim to resolve issues that may impact upon the quality of care reporting immediately to the line manager.

 

6: Equality and Diversity

•     Act in a way that supports equality and values diversity for both staff and patients, contributing to the development of a work culture that is reflective of this.

•     To work in such a way as to promote the privacy and dignity of patients.

•     Ensure service/care delivery reflects people’s rights, expressed beliefs preferences and choices acting in accordance with legislation, policies and procedures.

•     To recognise, report and record any situations where the cultural or spiritual needs of the patient are not able to be met or where there is an apparent injustice or inequality.

 

 

7. Provision of care to meet health and wellbeing needs

 

•     To encourage or support the client to meet their personal care needs

•     To support the client to meet their medical care needs.

•     To promote the health and well being the client through monitoring and reporting.

•     To deliver all planned care to a high quality standard being mindful of the clients cultural needs and provide feedback to the primary nurse the effectiveness of the care plan.

•     To enable the client to progress towards a greater level of independence.

•     To support / assist the client to access a wide range of leisure activities.
