An exciting opportunity has arisen for an Advanced Paediatric Nurse Practitioner/ Senior Nurse to work within Specialist Children’s Services and work closely with the Special School nursing team and Community Paediatric Team within Northamptonshire Healthcare Foundation Trust.

 

Prif ddyletswyddau'r swydd

·         Undertake Special School Review Clinics.

·         To work closely with Community Paediatricians to help support families during diagnosis to offer follow up home visits to ensure families are aware of all avenues of support for social, emotional and health needs and referring.

 ·         To work closely with Community Paediatricians to facilitate pathway for, and support children with an assortment of health conditions that are impacting on their wellbeing and school attendance.

·         Attend/participate/in Multidisciplinary Team Around the Child (TAC) meetings for complex children.

 ·         To facilitate and participate in transition planning for Children and Young People as required at key transition points.

 ·         Attending and participating in Child Protection Conferences/ Core Groups as required and writing reports for core groups.

 ·         To work flexibly, working across a variety of settings including schools, clinics and home visits

 ·         Able to identify and lead on relevant audits of service provision, including multiagency audit in conjunction with others

 ·          Able to engage with vulnerable children and work effectively in distressing and challenging circumstances.

 ·         Able to work flexibly and co-operatively as part of a team

 ·          Able to use own initiative and make decisions independently

 ·         Committed to continual quality and service improvement

 ·         Able to accept and respond positively to feedback

NHFT is an integrated primary care and mental health Trust, providing physical, mental health and specialty services in both hospital settings and out in the community. This means many of our services can be provided at home, work or in schools. We also provide health services to various prisons and detention centres in Bedfordshire and Cambridgeshire.

Swydd ddisgrifiad a phrif gyfrifoldebau manwl

  • Waiting List Management    -   To participate in the RTT process for the community paediatrics service
  • Clinical contact    -   To manage own caseload, facilitating face to face assessment, review and advise 
  • Telephone reviews    -   Telephone review and contacts are dealt with in a timely manner and queries escalated if necessary to relevant clinician
  • Outcome appointments    - Ensure that the outcome of all appointments is maintained and that the CYP is moved to the appropriate waiting list/discharged etc. Record keeping is completed as per Policy and on System One.
  • WNB/DNA Process    -  Ensure WNB/DNA process is adhered to and always followed, housekeeping of WNB/DNA waiting lists is monitored and actioned on a weekly basis to adhere to the Trust’s WNB Policy
  • SystmOne Task Management    -    Action tasks in a timely manner as per the Trust Task Management Group.  Tasks on SystmOne should not be open for longer than 6 months
  • Prescriptions and Non-medical prescribing    - As per Trust policy and adhere to audit process 
  • Team Meetings    -  Attend and participate in monthly team meeting. 
    Diary Management    -  Managing diary, organising meeting/appointments 
  • Other administrative tasks    -    Complete Datix as required
