Job overview

Are you looking for a new and exciting challenge? We have a fantastic opportunity for a passionate patient liaison lead to join our friendly team in Trauma and Orthopaedics.  To be a successful patient liaison lead you will be compassionate and professional with excellent interpersonal and communication skills.  You will be responsible for maintaining positive relationships with patients and their families and a link to health care professionals to ensure safe and effective care for our patients.  

This post will be a fixed term 12 month job share

Main duties of the job

The post holder will:

Work cohesively within the MDT to establish treatment plans and any future care needs of all elderly frail trauma patients.

Provide a point of contact to promote consistent and timely communication between the clinical care teams, patients and their families.

Work collaboratively with patients, families and the multidisciplinary team to allow early supported discharge and contribute to complex discharge planning.

Directly contribute to health promotion initiatives within the context of the frail elderly patient cohort – such as falls prevention, bone health and nutrition. Signposting patients and families to relevant resources.

Meet with patients to discuss the quality of their care and collect information to support departmental / organisational quality audits.

Recognise and address any anxieties and deescalate any concerns/complaints whilst seeking support from appropriate professionals where appropriate.

Create a positive experience by helping patients and their families feel comfortable and informed about their care.

Essential criteria
  • Prior experience in health care for frail elderly trauma patients.
  • A good understanding of the complexities of discharge planning and social care provision
  • Knowledge of medical terminology
  • Effective verbal and written communication skills
Desirable criteria
  • Prior experience in health care for frail elderly trauma patients at B3 level
