These posts cover NCHC Places - North and Norwich, 

These posts are offered as either Fixed Term/ secondment opportunities. Within your application, along with your preference for full or part time (with hours) state your preference for fixed term / secondment and which your 1st and 2nd preference for places would be.

Role overview

·        To have a contributory role in the delivery of Specialist Tissue Viability services in Norfolk, working autonomously but closely with the Skin Integrity specialist leads and Team Lead and members of the wider Skin Integrity Team, assisting the development of specialist and generalist clinical practice in Tissue Viability management.

·        To act as a specialist practitioner and expert resource for patients with tissue viability issues, their families and other healthcare professionals whilst working collaboratively with the wider multi-disciplinary team.  This will be to provide the best care and support to patients and their families.

  • To support the delivery of Tissue Viability education across teams within NCH&C

The post holder will:

  • Maintain tissue viability best practice across NCH&C in partnership with the relevant service leads to include community teams, dermatology and lymphoedema specialists. 
  •  Maintain a patient caseload, delivering and evaluating the effect of clinical /therapist specialist care and interventions.
  • To apply developed assessment, analytical and clinical reasoning skills in evaluating the patients’ condition and in progressing and executing appropriate treatment plans and maintaining records.
  • Expectation for staff to be place based with a requirement to support and cover other place clinicians during times of leave or high demand or as the service requires.
  • To deliver the agreed skin integrity education and training plan for community teams within NCH&C. 
  • Engage clinical teams in the implementation of new pathways of care to meet the requirements of national tissue viability guidance as and when it is published.
  • Provide information to support informed consent and act as the patient advocate when informed discussion may lead to choices being made with regard to care and treatment.
  • To work closely with the TV Nurse teams from the local / referring Acute Trusts around the discharge of patients with complex wounds; providing a seamless service for the patient.

The post holder will: 

  • Forge links between all health care settings across NCHC to maximise opportunities to improve patient care and the environment in which care is delivered; by maintaining a highly visible profile and through regular contact with clinical staff, patients and carers.
  • Support and develop audit within community teams in conjunction with wider Skin Integrity Team.
  • Proactively manage the Trust pressure ulcer action plan and associated processes in supporting the reduction of preventable pressure ulcers alongside TV and Skin Integrity Leads.
  • To provide wound care advice and support for community nursing teams to reduce the risk of patient harm, improve healing and therefore reduce the number of avoidable inpatient admissions. 

·  Actively contribute to multidisciplinary team meetings and other meetings where appropriate.

·    Develop and maintain effective working relations and communication with all members of the organisation, other professionals and agencies.

·        Contribute to working groups.

·    Support all team members within the service, respecting them, their roles and contributions.

·   At all times ensure that one’s own actions support and promote equality, diversity and the rights of patients, the public and colleagues within the healthcare environment. 

·        Be responsible for ensuring own health and safety whilst lone working following team processes and Trust Lone worker policy.

·      Will be able to drive to patient homes, clinics, office base and meetings etc. as required.

·  Be responsible for safe use and maintenance of equipment and supplies.

·    Manage standard and specialist resources in conjunction with the Team Lead and place based clinical teams, in an efficient and effective way, where necessary, influencing budgets through the use of expert knowledge and judgement.

·        Contribute to the audit process as required.

·        Remain accountable for own professional actions as determined by the professional bodies, and in addition the actions of less senior staff.

·        Participate in evidence based practice, using research methodology.

·  Mentor existing staff and provide training in specialist area as appropriate.

·        Demonstrate basic IT/standard keyboard skills.

·        Participate in Trust education programmes including learning hubs.
