Job overview
The Senior Nurse for hospital discharge will be a visible and professional clinical leader with responsibility for day-to-day discharge process while liaising with multi professional colleagues throughout the community hospitals to ensure timely complex discharge arrangements are in place.
The post holder is required to support and align with the Senior Nurses for community wards prioritising the implementation of national guidelines and escalation processes.
Responsible for supporting policy and service development and for the budget and physical resources for the hospital discharge teams.
Main duties of the job
Assess the ability of the community hospital sites to cope with escalation planning and devise and implement a daily operational plan
• Achieve a constantly up to date assessment of the hospitals known capacity and anticipated demands, and provision of this information to key people.
• Ensuring the delivery of the required standards and continual improvement of the fundamental aspects of safe care, cleanliness and infection prevention and control.
• Safe and effective use of staff resources on a day-to-day basis in accordance with need and appropriate skill level within the designated teams.
• Timely, safe and effective discharge for simple and complex discharges.
• Ensuring positive patient experience, reduction in harm, waste and variation. This includes leading digital
transformation and support nursing workforce to confidently work with innovative technologies.
• A culture that fosters empowerment, collaborative learning and continuous professional development.
• Actively supports the organisation’s ability to respond to change and continuously improve by fostering a
transformative approach to leadership style and management.
• To support the on-call senior nurse rota at Tier 1.
