Job overview

An exciting opportunity has arisen for an experienced, enthusiastic and highly motivated Band 2 HCSW to join our friendly and supportive team at Pillmawr Rehabilitation Ward. The post is for 37.5 hours per week working 3 x 12 hour shifts.  Mixture of days/nights and weekends. 

We are looking for staff who have energy, enthusiasm, commitment and drive and who are patient-centred.

You will have the opportunity to develop your skills under the supervision of our qualified nurses, caring for patients during their rehabilitation and in achieving their discharge with the best possible outcomes.

The post holder will work under the supervision/guidance of a registered nurse/clinical lead and will work collaboratively with them and the wider multi-disciplinary team (MDT).

Main duties of the job

The post holder will be expected to act at all times within the guidelines of the client’s care plan and within the supervision of senior support and or professional in charge

The post holder is expected to maintain positive relationships with service users, relatives, carers, members of the mental health team. They are also expected to work under the guidance of CPNs, Social Workers, Occupational Therapists, Dieticians, Psychologists Speech and Language therapists, District Nurses, Consultants and other medics.

The post holder will be expected to attend and fully complete an agreed induction programme and undertake on-going training relevant to the role. They will also have the option to achieve a Health and Social Care NVQ Level 3 or equivalent to support their career development.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Detailed job description and main responsibilities

To contribute to and be actively engaged in a value base service which meets the needs of service users, ensuring that their wishes to live and experience an ordinary life are respected

To support the senior qualified staff in the delivery of Care Plans for service users ensuring appropriate interventions in the Care Plans are delivered.

To contribute to the assessment of risk and need, liaising and sharing information, which may be complex and sensitive, with the more senior staff members involved in the service user’s care.

To support service users to engage effectively in the development of their Care Plan

Qualifications

Essential criteria
  • Health or Social Care related NVQ 2 or Equivalent Compentencies
  • Ability to form positive therapeutic relationships with service users

  • Supporting individuals in emotional or mental crisis
  • Experience of working paid or voluntary with people in the provision of social, mental or similar care to people who require assistance in their everyday life
