 Community Nursing Team Leader- Band 7   for Claughton  Community Nursing Team   

An exciting opportunity  has arisen for a  motivated individual to work within the Community & Specialist Nursing Division in the Birkenhead  Locality  as a Community Nursing Team Leader, within our team at Claughton  Community Nursing Team.

This is a permanent post.

Working over 7 days , must be flexible .

Main duties of the job

A main part of the role  is to assess, plan, implement and evaluate nursing care to patients across the community/primary care interface in the most efficient and clinically effective way, in co-operation with other members of the health and social care teams, and voluntary and statutory agencies.

This post will be responsible for managing planned programmes of care for patients in domiciliary and clinic settings.

Qualification and Professional

Essential criteria
  • Registered Nurse educated to degree level (or equivalent)
  • Current NMC registration
  • Independent non-medical prescriber or willing to undertake
  • Teaching/Mentorship Qualification
  • Evidence of further recent study/continuous professional development
  • Improvement practitioner qualification or willing to work towards
  • Post Graduate Leadership Qualification or willing to work towards or relevant experience
Desirable criteria
  • Educated to masters level or working towards/equivalent experience
  • Specialist Practitioner Qualification – District Nursing
