Prif ddyletswyddau'r swydd

CLINICAL

  1. To provide a high standard of nursing care to a specified client group.
  2. To fulfil the role of Care Coordinator for a caseload of patients residing in the community.
  3. To monitor restrictions under the terms of conditional discharge as set out by the Ministry of Justice.
  4. To fulfil the requirements of MAPPA for patients that this is required. Participate fully in the MAPPA process
  5. To provide continuous review of a caseload, reporting to MDT’s and the FOS Manager, utilising collaborative care planning.
  6. To attend clinical meetings at secure in patient units across Prospect LPC, outpatient department and any other areas as necessary.
  7. To observe confidentiality in the care of patients and their case records.
  8. To administer medication as prescribed to individual patients. To provide support for patients at home or in supported accommodation.
  9. To regularly attend the outpatient department for the assessment and follow up of patients.
  10. To report progress of the service to the FOS Manager.
  11. To fulfil legal requirements under the framework of Care program Approach (CPA)
  12. To assess the psychological needs of patients and address holistic needs of clients and their families as needed.
  13. Take and deliver blood and other sample/specimens as needed in line with service requirements/developments.

MANAGEMENT

  1. To participate in the development and organisation.
  2. To attend Multidisciplinary Team meetings.
  3. To attend nursing meetings and other meetings as necessary.
  4. To record all visit undertaken and to keep statistical records.
  5. To organise case conference and other meetings regarding clinical care in consultation with the Team Manager.
  6. To be aware of the legal rights of the patient under the Mental Health Act 1983/CPA /Other legal requirements.
  7. To maintain clear effective communications with the Secure inpatient units, secondary providers, FOS and the Trust.
  8. To liaise with other agencies involved in the care of the patient e.g. Probation Service, Social Services, GPs, local authority staff etc
  9. To liaise with and support patients’ relatives in the progress and care of patients in the community.
  10. To ensure that Patient’s records are kept correctly and up-to-date.
  11. To report any unusual adverse incidents to the FCS Manager, Nurse Manager, General Manager, Head of Service,
  12. Participate in “on-call” rotas as needed and to act up (or across) as required.

 

PERSONNEL AND TRAINING

  1. To provide educational opportunities via learning experiences to other unit staff and staff on placement.
  2. To deliver education and training to staff in the special needs of forensic psychiatry patients.
  3. To follow agreed policies laid down by the Trust/NMC regarding education of learners/others.
  4. Provide supervision to students on post basic educational or training courses e.g. Modular, Certificate, Diploma courses etc.

PROFESSIONAL

  1. To receive and provide clinical/managerial/professional/educational supervision and support as necessary or directed.

  • Hold, or be currently working towards clinical skill-based qualification e.g., PSI / CBT, counselling

Knowledge/Experience

  • Demonstrable substantial relevant clinical practice and care management experience gained within a management role
  • Evidence of experience of working with a range of people with mental health problems
  • Experience of Risk Assessment and Health & Safety Management
  • Experience of providing Clinical Supervision
  • Able to demonstrate flexibility in working hours e.g., participation in on-call rotas
  • Experience of change management
  • Minimum of 2 years post qualification-nursing practice in a Forensic Psychiatric setting
  • Facilitation of placements and supervision of junior staff
  • Knowledge of developments in forensic and community care and of mental health legislation
  • Knowledge of CPA
  • Demonstrate knowledge in the concepts of mentorship, preceptorship and supervision
