An  exciting  opportunity has arisen for  motivated individual to work within the Community Nursing Division as a Community Nursing Caseload Manager,  within the Wallasey Locality working with the Liscard  Community Nursing Team, based at Victoria Central Hospital, Mill Lane Wallasey.

Main duties of the job

This is a  permanent  contract .

Hours per week are 37.5, shift hours to be discussed at interview. 

This post will be responsible for managing planned programmes of care for patients in domiciliary and clinic settings.

A main part of the role of a Caseload Manager is to assess, plan, implement and evaluate nursing care to patients across the community/primary care interface in the most efficient and clinically effective way, in co-operation with other members of the health and social care teams, and voluntary and statutory agencies.

You will need to be able to work autonomously and as part of a team and possess excellent time management, organisational and communication skills. You will be innovative, flexible and prepared to adapt to the changing needs of the Trust as they arise.

Quals +Professional Training

Essential criteria
  • Registered General Nurse educated to degree level (or equivalent)
  • Current NMC registration
  • Specialist Practitioner Qualification – District Nursing (or equivalent postgraduate modules) or equivalent level nursing qualification/ experience
  • Evidence of further recent study/professional development
Desirable criteria
  • Independent non-medical prescriber or willing to undertake
  • Teaching/Mentorship Qualification
  • Clinical diagnostics module (or willing to undertake)
  • Clinical examination module (or willing to undertake
  • Leadership Qualification – ILM level 6 or equivalent (or willing to work towards) or relevant experience

Experience and Knowledge

Essential criteria
  • Experience of managing a caseload/leadership within the community
  • Knowledge and experience of managing patients with long term conditions
  • Knowledge of tissue viability, including pressure and leg ulcer management
  • Knowledge and experience of end of life care
  • Knowledge and experience community nursing conditions
  • Awareness of Workforce Policies and Procedures
  • Evidence/experience of teaching/mentoring
  • Experience of Multi-disciplinary working
  • Supervisory and/or undertaking clinical supervision experience
  • Awareness of professional issues and developments
  • Understanding of national government initiatives in health and social care and how they impact on local services
  • Experience of clinical assessment, analysis, planning, delivery and evaluation skills
  • Ability to manage complex cases
