An excellent opportunity has arisen for an Advanced Clinical Practitioner to join our team on Ward 7A within the Department of Medicine for Older People in Liverpool University Hospitals Foundation Trust.
This is not only a brilliant opportunity for experienced ACPs, but we also welcome applications from trainees. To consolidate their learning and development, with it culminating in their qualification as a fully-fledged ACP.
The successful candidate will work closely with their peers, consultants, members of the senior nursing team and their colleagues in other specialities. Providing specialist input and education, alongside service development and audit work.

Main duties of the job
To be part of our team you will be expected to always maintain a professional and non-judgemental attitude. Whilst providing our patients and relatives and or carers with expert care, advice and support. Demonstrate compassionate, clear and concise communication to our patients, relatives and or carers, members of the team and the wider clinical teams. To act as an agent for change and service development through audit and education.
As the population ages increasing numbers of older people are presenting to ED departments with a wide range of issues in addition to their acute problem. The Gerontology wards often deal with multiple issues alongside our patients' acute needs, making this role both challenging and rewarding on various levels.
The successful candidate will part of a busy but rewarding department and will be supported with their personal and professional development.

Detailed job description and main responsibilities
Main Responsibilities -
1. To maintain personal accountability for professional and ethical actions and ensure compliance with the standards of professional conduct issued by the relevant regulatory body
- To maintain and develop a specialist service for patients within the caseload, ensuring that the treatment and follow up of this group of patients is delivered in a timely manner.
- To provide and maintain a diagnostic service for the Trust. Ensure clinician led clinics provide for both diagnoses and reporting of subsequent results for identified patients.
- To provide teaching as an in-house measure to all medical and clinical staff, not only within the Care Group but also from other services.
- To provide a clinician-led clinic service to patients.
- To plan and administer treatment to patients as appropriate and ensure handover of this treatment to community services as necessary.
- To provide expert care, advice and support for patients and them relatives/carers and ensure that advice and point of contact is available throughout the patient’s journey.
- To refer on to other agencies as appropriate.
- To operate as clinical specialist and expert practitioner in issues relating to the identified area of care. To direct and delegate care based on accurate assessment and evaluation.
- To take consent for investigative procedures, manipulate equipment and perform advanced clinical skills.
- The post holder will be expected to work clinical shifts in a ward environment if necessary
Management and Quality.
- Act as a change agent for the development of the service.
- Assisting in the audit and data collection required for developing evidence based services.
- To provide verbal and written reports to appropriate managers and clinical leads.
- To monitor the effectiveness of the service by continuous audit.
- To ensure that equipment is maintained and serviced as required.
- To develop protocols, documentation, standards and guidelines to enable best practice for the care of patients. In the identified/specified area.
- To be involved in complaints handling according to the Trust’s Complaints Procedure.
- To establish a clear focus, vision and direction for the service, within the Care Group.
- To ensure effective and efficient use of financial resources, contributing to increasing the efficiency and controlling cost within the departmental budget. Making recommendations regarding supplies and purchasing of equipment.
Communication and Liaison
- To promote effective communication within the hospital and to the patient and/or their relatives.
- Act as a point of liaison between all hospital departments involved in the patient’s journey.
- Act as a source of specialist information for clinicians and other members of the multidisciplinary team.
- Liaise with other departments/ Trusts regarding services provided and current practises to develop best evidence-based practice.
- To be skilled to impart distressing information with respect and empathy.
- To ensure that all patient records are maintained contemporaneously.
* The post holder shall as necessarily provide cover for and undertake duties of absent colleagues.
* The post holder shall follow all the policies and procedures of the organisation.
Person specification
Qualifications
Essential criteria
- Clinical Professional Qualification. Degree level with appropriate registration
- MSc Advanced Clinical Practice (ACP) or started the MSc ACP pathway/programme and committed to completing within an agreed time frame: Note: Post holder will be in a training post (Annex 21) until evidence of completion of MSc ACP is provided
- Teaching and assessing qualification
- Relevant post registration qualification/equivalent.
