Caseload Coordinator - some weekend working required
This is an exciting time to join our team, supporting us to care for patients safely in their own home avoiding unnecessary admissions and facilitating early discharges from hospital.
UCR/VW is a multi-disciplinary team that provides a two-hour urgent response to patients in their own home that are experiencing a crisis The service can clinically monitor patients remotely via Virtual Ward/Hospital at home using enhanced technology which is supported by a Digital Health team.
Main duties of the job
This role will involve managing the case load of clinical staff within the Urgent Community Response (UCR) and Virtual Ward (VW) Team. It will involve gathering, inputting and providing essential data for patients, helping to maintain accurate records and enhancing the team’s ability to provide personal care.
The role is responsible for the administration support and caseload co-ordination within the team to support the delivery of care to patients. The post holder would also be required to answer the telephone, which will include calls from patients, families, carers and other service users, accurate information will need to be recorded and forwarded onto a clinician.

Detailed job description and main responsibilities
- Ensure that the team case load is kept up to date.
- To participate and co-operate with the Multi Disciplinary Team to contribute to the effectiveness of the service as a whole.
- To manage and maintain the team’s clinical caseload, including all associated administration tasks.
- To use multiple patient information management systems (EMIS, Docobo, Patient Centre etc.) to enter such information as required on the computerised patient information system.
- To use discretion, sensitivity and deal with information in a confidential and professional manner at all times.
- Oversee and monitor the distribution/collection of equipment that is used to monitor patients in their own home (tech-enabled care).
- Maintain an accurate database of the home monitoring devices.
- To be responsible for data collection, based on the outcomes from the clinical caseload.
- To participate in and organise team audits when appropriate.

If successful this vacancy will require an standard DBS check at a cost of £21.90. This cost will be deducted from your first Trust salary
In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £13.

Qualifications
Essential criteria
- Good general level of education, including English and Maths or equivalent qualifications or equivalent experience
- NVQ Level 3 or above in a relevant subject such as Customer Care, IT, or Business Administration or equivalent experience.
Knowledge
Essential criteria
- To have an understanding of confidentiality
- Intermediate IT Skills including Microsoft Outlook, Word and Excel
Desirable criteria
- To have an understanding of current issues affecting the NHS and Social Care.
- Knowledge of rota systems
- Knowledge of Electronic Patient Record Systems

Experience
Essential criteria
- Experience of working as a team and independently
- Experience of working in a pressurised environment
- Relevant experience of providing a comprehensive clerical / administrative service
- Familiar with office procedures and deadlines
Desirable criteria
- Previous experience of a co-ordinating job role
- Experience of a variety of computer based programmes.

The postholder will have access to vulnerable people in the course of their normal duties and as such this post is subject to the Rehabilitation of Offenders Act 1974 (Exceptions) Order 1975 (Amendment) (England and Wales) Order 2020 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service to check for any previous criminal convictions.
