The Community Mental Health Team at The Meadows is looking to recruit a motivated and enthusiastic Registered Mental Health Nurse to join our supportive multidisciplinary team and contribute to this exciting endeavour. The successful candidate will have significant experience working with people with enduring mental health problems across a range of services.  

You will receive regular supervision and your personal development priorities will be shared and supported through our appraisal process.

As this team covers the whole of the trust’s footprint, the postholder must have a driver’s licence and access to a vehicle.

The post-holder will ensure the highest standard of nursing care provided in accordance with the policies and procedures of Pennine Care NHS Foundation Trust. 

The practitioner will provide interventions to service users within the CMHT and will work with the multidisciplinary team to offer a team approach to supporting service users in the community or for those on the discharge pathway from inpatient settings.

To contribute to case formulation and treatment plans.

To contribute to service development in collaboration with the wider team.

To participate in the education programmes of nursing students and other 
disciplines.

• Professional qualification Mental Health Nursing

• Registration with HCPC / NMC

• Experience in community mental health work

• Willingness to undertake further training relevant to the practitioner role resulting from legislative or service changes, as appropriate.

• Experience of supervision

• Experience of some crisis work/interventions

• Knowledge of CPA policy

• Experience in working with people with moderate to severe mental health needs

• Extensive knowledge / experience in social inclusion and recovery model of care

• Knowledge of mental health legislation

• Experience of MDT working

• Ability to care coordinate and be responsible for a caseload

• Ability to work effectively as part of a MDT

• Ability to supervise junior staff / students

• Effective time management

• Excellent written and verbal communication skills

• Recording relevant documentation to a professional standard

• Ability to communicate informally and develop supportive relationships with clients, carers and other professionals

• Utilise the supervision process

• Ability to value diversity and respond constructively to discriminatory behaviour

• Ability to provide professional expertise in the care of mental health service users

• Ability to provide appropriate education and training to peers and assistant as appropriate

• Computer literacy

• Flexible and adaptable

• Use of a car or access to a means of mobility to travel across the Trust footprint in line with service needs

• Willing to carry out all duties and responsibilities of the post in accordance with the Trust’s Equal Opportunities and Equality and Diversity policies

• Appointments to regulated and controlled activities require an enhanced DBS disclosure

  • Experience working in an MDT

  • Knowledge of least restrictive practice
  • Effective risk assessment and risk management skills
  • Ability to case manage a caseload of service users using rehabilitation principles
