Job overview

An exciting opportunity has arisen for a newly qualified or experienced mental health nurse, with a 
keen interest in older people’s mental health and dementia, to join the Oldham Memory Assessment 
Service on a 12-month secondment. The successful candidate will use their evidence based clinical 
skills to carry out complex memory assessments and support those with a suspected/ confirmed 
diagnosis of dementia, their families, and carers. You will carry out initial assessments and develop 
person centred care plans for patients with dementia and complex needs. You will work within an 
experienced and established multi-disciplinary team, delivering high quality patient care, and 
supporting people to access an early diagnosis and to live well with their dementia. This includes 
supporting those with early onset dementia.
Knowledge and skills of working with older people is preferred, as well as an understanding and/or 
experience working with people living with dementia. Experience of working in a multi-disciplinary 
team and developing good working relationships with other services is required, and willingness to 
develop their knowledge and skills and clinical professional development.

Main duties of the job

Undertake complex assessments both in the community and in clinical settings, using various 
evidence based cognitive assessment tools.
Provide specialist nursing knowledge and skills to patients referred to Oldham Memory Service.
Promote and develop evidence-based practice working within NICE guidelines and national dementia 
standards.
Work effectively within the multi-professional team to deliver high quality care.
Actively participate in various meetings and service development.
Actively participate in facilitating the post diagnostic support groups and weekly dementia hub.
Offering advice and information to patients and carers both in person and on the telephone.
Monitoring and titration of medications initiated to treat dementia following a diagnosis.

Working for our organisation

Oldham Memory Assessment and Treatment service provide a specialist assessment, diagnosis and 
treatment service for people living in Oldham concerned they might be developing dementia. The 
primary aim of the service is to provide access to an early diagnosis and promote living well with 
dementia, as well as providing support to the patient, their family, and carers. We provide emotional 
and practical support to help people live well with dementia and offer a range of post diagnostic 
support services. We strive to provide excellent standards of dementia care and the whole team is 
passionate about improving lives of those people affected by dementia.

education/qualifications

Essential criteria
  • First level registration. Registered Mental health nurse
  • Current registration with Nursing and midwifery Council
Desirable criteria
  • Post registration development in Older Peoples mental health.
  • Mentorship qualification or willingness to attain

Experience

Essential criteria
  • A broad range of clinical skills or demonstrate willingness to develop.
  • Experience of working in a clinical setting with people with dementia
  • Experience of completing specialist mental health assessment and risk assessment / management
  • Using therapeutic assessment techniques and interventions in the management of behaviour that challenges in dementia
Desirable criteria
  • Multi agency working
  • Practical application of mental Capacity Act including completing mental capacity assessments.

Knowledge

Essential criteria
  • Clinical and theoretical knowledge of older people and mental health problems
  • Need to demonstrate a genuine interest in older people and be highly motivated to develop the service.
  • A detailed knowledge of mental health conditions and how they impact on the individual.
  • Knowledge of legislation and national policies in relation to older people
Desirable criteria
  • Awareness of the holistic needs of older people with mental health problems.

Skills and Abilities

Essential criteria
  • Ability to undertake assessments, formulate care plans and review medication
  • Positive attitude and values when working with older people
  • Effective verbal and written communication skills.
  • Ability to work as part of a multi-disciplinary team with positive attitude

  • Work with third sector organisations / multiagency working
