To be a Primary Care Mental Health Practitioner within NHS Oldham Talking Therapies.
To work within a Multi-Disciplinary Team with the responsibility for providing full psycho-social assessment and brief interventions with mild to moderate mental health difficulties, to clients that are referred and accepted into our service. Assessment and interventions can be delivered within a clinic or as indicated by the intervention plan as required.
The service provides stepped care Talking Therapies with the addition of a commissioned step '3.5' element. Within the step 3.5 pathway we deliver a successful emotional skills group based on DBT principles. You would after training be expected to contribute to the delivery of this group.

To provide a thorough psycho-social assessment of the client (aged 16+), this will include a risk assessment and a care plan.
To undertake any other reasonable duty, which is appropriate to the band, when requested by Senior Staff To conduct a thorough psycho-social assessment of clients referred to Oldham Talking Therapies to identify their needs and identify collaborative care plan.
This joint working could take the form of supporting the in-vivo exposure element of a client’s therapy plan, facilitating attendance at appointments or behavioural activation plans, facilitating harm reduction procedures, etc.
To support the engagement and access of treatment for clients from vulnerable backgrounds or communities that traditional struggle to make use of our service, e.g. ethnic minority groups, young people, those with co-morbid substance misuse, troubled families or clients accessing Early Intervention support
To assess clients presenting with multiple and complex issues and make decisions regarding the appropriate therapeutic intervention required to meet client’s needs. This may include referral on to another service or modality of treatment.
To assess risk of harm to self / others or property and take action as appropriate. This may involve breaching of client confidentiality in the interest of the clients or others safety e.g. Safeguarding Children / Adults, acts of terrorism etc. This will be assessed in line with Pennine Care NHS Foundation Trust policies.

To deliver individualised interventions based on individual care plan
Liaison with all referrers, members of the Primary Care Mental Health Service, Secondary Care Mental Health Services, and Voluntary or Statutory agencies as required
To manage own caseload by delivering brief interventions to the client, whilst maintaining and allocating new clients from the Primary Care Mental Health Practitioner waiting list
To deliver agreed clinical contacts as set by service
To use Information Technology to support the care process to benefit service users and their families through recording relevant data. Record patient activity and interventions in a contemporaneous manner whilst acting on this information if necessary
To work autonomously within professional guidelines and code of conduct,
Pennine Care NHS Foundation Trust policies and procedures.
To be aware of and work towards the Trusts strategic goals
To undertake any other reasonable duty, which is appropriate to the band, when requested by Senior Staff

- Worked within a community setting
- Worked within a multidisciplinary team
- Managed a community client caseload
- Delivered a variety of therapeutic interventions within a community setting
- Full participation in clinical / managerial supervision
- Must be able to travel to a variety of locations and clients homes to deliver interventions
Knowledge

- Requires excellent knowledge around risk management
- Demonstrate up to date knowledge of current issues/legislation within Primary Care Mental Health including Talking Therapies model
