Job overview

The practitioner will assist in the utilisation of an early warning scoring system and responding to requests to provide a specialised assessment of patients who are deemed to be at risk of developing critical illness.

 The post holder will be an autonomous practitioner, continually developing a highly specialised knowledge base and clinical assessment skills, working within agreed protocols, as part of a multi professional team, supporting staff in the management of patients identified as, at risk of clinical deterioration.

 Where appropriate the post holder will liaise with medical colleagues to assess, implement, and modify treatment, or to facilitate timely admission to the critical care area.

 Patients in critical care will be assessed prior to discharge to the general wards, and appropriate guidance and support provided to the multi-disciplinary team.

The post holder will also play a key role in the delivery and development of formal and informal education to the members of the multi-disciplinary team.

Main duties of the job

·         To undertake specialised clinical assessments of patients who are at risk of clinical deterioration and are referred to the outreach service by the multi-disciplinary team.

·         The post holder will be an autonomous practitioner, continually developing a highly specialised knowledge base and clinical assessment skills, working as part of the multi-disciplinary team, supporting staff in the management of patients at risk of clinical deterioration.

·         Utilising critical care knowledge and skills and knowledge of evidence-based practice to provide specialist advice and support for individuals or multi-disciplinary team in the care of patient’s level 1-3.

·         The post holder will liaise with medical colleagues to assess, implement, and modify treatment, or facilitate timely admission to critical care .

·         Assess patients in critical care prior to discharge to wards and give appropriate guidance to the multi-disciplinary team.

·         To maintain high standards of care for acutely ill patients.

·         To respond to spontaneous requests for specialist advice, support and guidance from individuals or teams

·         To facilitate the timely admission of patients to the critical care areas, coordinating the different members of the multi-disciplinary team required to ensure a safe transfer.

·         Take responsibility for holding critical care bleep, responding to emergency calls from wards regarding critically ill patients.

This vacancy is open to those who have a current NMC PIN to practice as an Registered Nurse or Midwife in the UK, or those who are currently enrolled on a course which means they expect to receive their full NMC PIN within the next 3 months. Please note an NMC PRN PIN is not a full NMC PIN and would not fulfil the essential criteria for this role.

Person specification

Essential

Essential criteria
  • Registration with NMC
  • Post-basic qualification to minimum degree level or equivalent experience
  • Post basic qualification in Critical Care/Acute Respiratory care
  • Recognised Teaching qualification
  • ILS/ALS (Resuscitation Council UK) provider
  • Substantial Band 6 Level or substantial senior Band 5 level post registration experience in a high care speciality at, with knowledge and clinical expertise
Desirable criteria
  • Non-Medical prescribing course
  • Clinical Skills and assessment course
  • Current ALS provider/or willingness to undertake the course
