The post holder is a member of an integrated multi-agency learning disabilities team (Health and Social Services). The team provides a specialist service to adults and older people with learning disabilities in a range of community environments across Camden. Camden is a culturally diverse borough with many multi-ethnic, bilingual and multi-lingual speaking communities. These may provide potential barriers to communication which could affect delivery of client care and the post holder will need to have an awareness of, and sensitivity to, social, cultural and economic issues.

Prif ddyletswyddau'r swydd

·       Assess the health needs of people with learning disabilities, plan, implement the treatment and monitor care, providing highly specialist advice to services users, paid carers and family members

·       Manage a caseload of service users with complex needs (physical and mental health

·       Provide advice on assessments such as mental capacity and best interest

·       Develop effective and collaborative working with health and social care, multiagency colleagues, families and carers, including identifying and meeting the training needs and providing specialist and nursing advice

·       Incorporate the philosophy outlined in “Valuing People Now” and the principles embedded within the overarching structures as set within the Learning Disability Strategy, clinical governance framework and Disability Equality Scheme.

·       Supporting CLDS social work colleagues to carry out Safeguarding investigations for people with learning disabilities.
